Survive until night 5. On night 5, hang a right and there should be a door with a sign on it that says "Open." Interact with the door, then you'll be in the nightclub part of the nightclub. Talk to Puppet, she will give you a list of things to do, do them in a certain order (look for yellow highlighted objects),  then talk to her again once you have completed all the objectives.

How to I claim my reward after Puppet mission on might 5? I saw a key icon but nothing I could do with it.
