Abstract
Description
The invention relates to medicine, and more specifically to ophthalmology, and can be used for the surgical treatment of progressive and complicated myopia.
Progressive myopia continues to be one of the most urgent problems of ophthalmology, because, despite the undoubted successes achieved in recent years in prevention and treatment, this disease often leads to the development of irreversible changes in the fundus and a significant decrease in vision at a young working age. With the progression of the process, the scleral membrane stretches, its strength properties decrease and, as a result, circulatory disturbance in the vascular system of the eye, including in the posterior ciliary arteries. This is one of the links in the pathogenesis of chorioretinal dystrophies.
д.м.н. - М., 1993. - 51 с.).Studies have shown that with progressive myopia, more than 40% of children and adolescents develop complications - peripheral and even central dystrophies, the frequency of which increases significantly with increasing age, the degree of myopia and the duration of its course (EP Tarutta. Sclerotherapy treatment and prevention of complications progressive myopia in children and adolescents: Abstract of thesis, MD, MD - M., 1993. - 51 p.).
The most effective methods of treating progressive myopia are surgical - sclerotherapy.
Known methods of surgical treatment of progressive myopia using biological implants: auto- (Eroshevsky T.I., Panfilov N.I. Meridional strengthening of the sclera with wide fascia of the thigh with progressive myopia. // Bulletin of Ophthalmology. - 1970. - No. 2. - S. 19-23.), Allo- (Fedorov S.N., Ivashina A.I., Balashova N.Kh., Kuznetsova N.P. Prevention of progressive myopia by the method of scleroplasty: Methodological recommendations. - M., 1991. - S. 8.) (including bref- (Zaikova M.V., Lyalin A.N. Brephoscleroplasty with progressive myopia. // Ophthalmol. Journal - 1984. - No. 8. - S.463-467)), xeno-tissue (Cheglakov V.Yu. Xenoscleroplasty of the posterior pole of the eye in the treatment of patients with progressive myopia: Dis .... k.m.s. - M., 2006. - P.41-44). The disadvantages of the methods of surgical treatment of progressive myopia using biological materials are: difficulties in obtaining (associated with the lack of a clear legal framework for the removal of cadaver organs and tissues) and processing, moral and ethical and religious restrictions on their use, as well as the replacement of biological material with connective tissue over time the recipient, which reduces the stabilizing effect of the operation.
Known methods of surgical treatment of progressive myopia using synthetic implants: Teflon (Tarutta E.P., Iomdina E.N., Andreeva L.D., Markosyan G.A. Morphological features of the engraftment of synthetic grafts after scleroplasty in the experiment. // Herald of Ophthalmology . - 1999. - No. 3. - P.16-17), Mersilen (Tarutta E.P., Iomdina E.N., Andreeva L.D., Markosyan G.A. Strengthening of the sclera with new types of synthetic materials with progressive myopia. / / Bulletin of Ophthalmology. - 1999. - No. 5. - P.8-9), scleracode (Tarutta E.P., Iomdin a E.N., Andreeva L.D., Markosyan G.A. Morphological features of the engraftment of synthetic grafts after scleroplasty in the experiment./ Vestnik Ophthalmology. - 1999. - No. 3. - P.16-17), metal implants (author's certificate No. 1114417), silicone rubber (these implants are taken as a prototype) (Utkin V.F. Siliconoscleroplasty with progressive myopia in children and adolescents. // Bulletin of Ophthalmology. - 1987. - No. 3. - P.54-56). The disadvantages of the methods of surgical treatment of progressive myopia using synthetic materials: do not have a biologically active effect on the membranes of the eye, in some cases, the implant can be displaced; at the end of revascularization, the metabolic effect of the operation decreases, which does not exclude further progression of the dystrophic process, despite the stabilization of the anatomotopographic parameters.
One of the promising methods of surgical treatment of progressive and complicated myopia may be the use of mesenchymal stem cells (MSCs), however, such methods are not yet sufficiently developed.
В офтальмологии терапевтический потенциал стволовых клеток изучался на животных с наследственной ретинальной патологией, близкой к пигментному ретиниту человека (Lund et al. Subretinal transplantation of genetically modified human cell lines attenuates loss of visual function in dystrophic rats. // PNAS. - 2001. - V.98. - N.17. P.9942-9947; Rander et al. Sci - 2003. - V.44. - P.1686-1695).Stem cells have a number of significant advantages: they can provide regeneration of damaged areas through the production of various growth factors and key metabolites; able to deploy proliferation and differentiation programs, thereby filling the lack of actively working cells; can integrate into pathologically altered areas of the retina and form cells with a retinal phenotype. In ophthalmology, the therapeutic potential of stem cells has been studied in animals with hereditary retinal pathology close to human retinitis pigmentosa (Lund et al. Хотя мезенхимальные стволовые клетки взрослого организма обладают более ограниченным потенциалом дифференцировки, чем эмбриональные стволовые клетки, получаемые при культивировании клеток бластоцисты, их применение более безопасно. Although adult mesenchymal stem cells have a more limited differentiation potential than embryonic stem cells obtained by culturing blastocyst cells, their use is safer. In addition, from an ethical point of view, they are more acceptable for clinical use.
An essential problem of cell therapy is the targeted delivery of stem cells to the pathological focus and their retention in it for the time necessary to achieve a therapeutic effect.
The objective of the invention is to provide an effective method for the surgical treatment of progressive and complicated myopia.
The technical result of the invention is the strengthening of the strength-elastic properties of the sclera, stabilization of the myopic process with the simultaneous prevention of the development of dystrophic changes in the fundus or further progression, if any.
The technical result is achieved due to the fact that:
1. Use a three-component complex containing MSCs labeled with magnetic microparticles, translocated into a biological or synthetic finely porous material, which is firmly bonded to a polymeric magnetic material with a constant magnetic field induction of 1.5 mT, with multi-pole reverse magnetization.
2. The translocation of MSCs labeled with magnetic microparticles into a biological or synthetic finely porous material in combination with tight attachment to a polymer magnetic material with the induction of a constant magnetic field of 1.5 mT, with multipolar reverse magnetization ensures deep penetration of MSCs into layers of a biological or synthetic finely porous material, retention and uniform distribution of MSC during implantation of a three-component complex to the posterior pole of the eye for the time required for stizheniya therapeutic effect, due to the interaction of magnetic fields and magnetic material microparticles.
3. The implantation of a three-component complex to the posterior pole of the eye is accompanied by directed selective adhesion of MSCs in this area, which contributes to the activation of reparative processes due to the engraftment of transplanted stem cells, accompanied by the strengthening of the strength-elastic properties of the sclera due to the formation of a fibrous capsule around the complex.
(Lamba D.A., Karl M.O., Ware C.B., Reh T.A. Fiedlander M. Fibosis and diseases of the eye. // J. Clin. The possibility of such processes for embryonic stem cells and adult stem cells has been shown in animal experiments (Lamba DA, Karl MO, Ware CB, Reh TA Efficient generation of retinal progenitor cells from human embryonic stem cells. // PNAS, 2006, v. 103, n. 34, pp. 12769-12774. Fiedlander M. Fibosis and diseases of the eye. // J. Clin. Invest., 2007, v. 117, n.3, pp. 576-586), although many of the mechanisms for the implementation of the therapeutic effect are not fully understood.
The method is as follows. At the preliminary stage, a three-component complex is prepared containing MSCs labeled with magnetic microparticles, translocated into a biological or synthetic finely porous material, which is bonded to a polymer magnetic material with a constant magnetic field induction of 1.5 mT, with multi-pole reverse magnetization.
MSCs are grown in a culture of bone marrow cells taken from a patient during a diagnostic puncture from the sternum or ilium (volume 0.5-1.0 ml). Cultivation is carried out in a special box for cell cultures using the following equipment - a centrifuge with 50 ml disposable sterile centrifuge tubes, an air thermostat, a laminar box, inverted and conventional microscopes, automatic pipettes, carbon dioxide and air cylinders, Goryaev cameras for concentration calculation cells. For the cultivation of the cells of the original bone marrow using sterile disposable plastic culture bottles with a bottom area of 25 and 150 cm2. When reproducing MSCs, the following media and solutions are used: RPMI-1640 medium, medium 199, antibiotics: penicillin, amphotericin, L-glutamine solution, fetal calf serum. For 12-14 consecutive doublings (within 25-30 days) from the initial number of undifferentiated MSCs contained in the obtained puncture bone marrow puncture of the patient and amounting to approximately 103 cells produced approximately (1-2) × 107 MSCs required for successful stem cell transplantation.
95%.Magnetic particles (d = 2.8 μm) are introduced into the cytoplasm of mesenchymal stem cells according to the following procedure. Upon reaching 80-90% confluence, a suspension of magnetic particles is added to the MSC culture. I pretreat magnetic particles with surfactants to create conditions for penetration into the cell cytoplasm. Cells are incubated with particles for 24 hours in a CO2 incubator. After incubation, the culture medium is changed and the cells are washed 5 times from free magnetic particles with Hanks solution. The efficiency of MSC labeling with magnetic particles by this technology is about 90%. Stem cell viability is 95%.
The finished three-component complex should be in the form of a flat blade 16 mm long, with a cloth having the structure "MSC - biological or synthetic finely porous material", 13 mm long and an area of 50-65 mm, intended for implantation to the posterior pole of the eye, and a leg devoid of structure “MSC - biological or synthetic finely porous material”, up to 2 mm wide and 3 mm long, intended for stitching the implant to the sclera. The implant web may have an oval, or rectangular, or trapezoid shape, for example, isosceles. A rectangular and trapezoidal web can be made with rounded corners for easy implantation. The implant leg may be, for example, semi-oval or rectangular. The rectangular leg can be made with rounded corners.
Намагничивание полимерного магнитного материала может быть произведено, например, как описано в патенте РФ №2187162.To do this, a blank is made in the form of a flat blade with a blade and a leg made of a polymer magnetic material of the samarium-cobalt or neodymium-iron-boron system with a constant magnetic field induction of 1.5 mT with multipolar reverse magnetization. The thickness of the magnetic material is 0.3 mm. The magnetization of the polymer magnetic material can be produced, for example, as described in the patent of the Russian Federation No. 2187162.
From biological material, for example, dura mater or sclera, or collagen, or alloplant, or from synthetic finely porous material, for example, hydrogel or digel, or polyester fabric, or silicone sponge, etc. perform a workpiece corresponding to the canvas of a flat blade of a polymer magnetic material.
MSCs, labeled with magnetic microparticles, are layered on the surface of the workpiece from a biological synthetic finely porous material. Then, an external magnetic field with a strength of 30 mT is fed under a Petri dish with MSCs labeled with magnetic microparticles. In this case, the size and shape of the external magnet corresponds to the size and shape of the workpiece from biological or synthetic material. Thus, between the external magnet and the MSC containing magnetic particles is a biological or synthetic finely porous material. MSCs containing magnetic particles quickly and deeply translocate into layers of a biological or synthetic finely porous material under the influence of a magnetic field. Their fastening and spreading occurs within 1-1.5 hours after application. The fixed cells are firmly retained in the layers of the porous material even in the flow of the culture fluid, which was modeled experimentally using a peristaltic pump. Cells containing magnetic microparticles proliferate, completely covering the surface of a biological or synthetic finely porous material, which proves the preservation of the functional properties of MSCs.
In conclusion, the resulting structure “MSC - biological or synthetic finely porous material” is firmly fastened, for example, by suture fixation or biological glue with a workpiece made of a polymer magnetic material.
Scleroplastic surgery using a three-component complex is performed as follows. In the lower and upper outer quadrants, retreating from the limb by 6 mm, the conjunctiva and tenon shell are cut 8-9 mm long, using the spatula, two tunnels are formed towards the posterior pole of the eye so that the three-component complexes fit freely in them. Each three-component complex is immersed under the tenon membrane with the distal part to the posterior pole of the eye and fixed to the sclera 10 mm from the limbus by the leg. The operation is completed by suturing the conjunctiva and the introduction of an antibiotic under it.
