Abstract

The invention relates to an osteosynthesis helping device, i.e. an osteosynthesis plate (4), which can be firmly connected to a medullary pin (3). At the same time, it relates especially to an osteosynthesis helping device for femoral neck fractures, comprising a medullary pin (3) for the proximal region (A) of the femoral bone, a connection provided on its outer end for an osteosynthesis plate, and an osteosynthesis plate having bores for the reception of bone screws (6, 2), one set of bone screws (6) serving to fix the osteosynthesis plate on the femur and the other set of bone screws (2) serving to connect the osteosynthesis plate (4) to the neck of the femur, and, furthermore, the bores having a transversal spacing which is larger than the diameter of the medullary pin (3). It is also possible to fix an osteosynthesis plate to the head of the humerus by means of screws to which a medullary pin can be firmly connected. <IMAGE>

An osteosynthesis aid, i.e. an osteosynthesis plate that can be fixedly secured to an intra medullary nail is disclosed, in particular an osteosynthesis aid for treating pertrochanteric fractures of the neck, of the femur with an intra medullary nail for the proximal area of the femur having at its outer end coupling means to an osteosynthesis plate. The osteosynthesis plate has bores for receiving bonFt screws (6, some bone screws secure the osteosynthesis plate to the femur and other bone screws link the osteosynthesis plate to the neck of the femur. The bores are transversely spaced apart at a distance larger than the diameter of the medullary nail It is also possible to secure by means of screws to the head of the humerus an osteosynthesis plate to which an intra medullary nail can be fixedly secured.
(57) Zusammenfassung Die Erfindung bezieht sich auf emn Osteosynthesehilfsmittel, d.h. eine Osteosyntheseplatte die fest mit einem Marknagel verbindbar ist. Hierbei bezieht sie sich insbesondere auf emn Osteosynthesehilfsmittel bei pertrochantemn Oberschenkelhalsbruch mit einemn Marknagel filr den proximalen Bereich des Oberschenkelknochens, mit einemn an seinemn Iusseren Ende vorgesehenen Anschlhss for eine Osteosyntheseplatte sowie einer Osteosyntheseplatte mit Bohrungen zur Aufnahme von Knochenscbrauben wobei die einen Knochenschrauben zur Festlegung der Osteosyntheseplatte am Oberschenkel dienen und die anderen Knochenschrauben zumn Verbinden der Osteosyntheseplatte mit demn Schenkelhals und wobei weiterhin die Bobrungen einen transversalen Abstand aufweisen, der gr~5sser ist als der Durchmesser des Marknagels Auch ist es m~glich, am Oberarmkopf eine Osteosyntheseplatte durch Schrauben festzulegen, an der fest emn Marknagel anschliessbar ist.
1 OSTEOSYNTHESIS AID FOR A TRANSTROCHANTERIC FEMORAL NECK FRACTURE The present invention relates to an osteosynthesis aid for I bone fractures and in particular, but not exclusively, for use in the case of a transtrochanteric femoral neck fracture or in the case of an upper arm humeral fracture.
A special osteosynthesis aid was proposed in the American Patent US-PS 47 33 654, in which an additional portion of intramedullary nail is fastened to the proximal region of S. 10 the intramedullary nail, said portion having a hole drilled I for accommodation of an especially long bone screw. The osteosynthesis plate is here configured with an L-shape and S'it grips into this auxiliary portion of the intramedullary nail by way of its short upper obliquely-directed shank.
This osteosynthesis aid is thus forced into the ;1 intramedullary nail because of the drill id hole provided 'i i and, in the auxiliary component, determines firmly established directions of the screws, that is to say, the Sattachment and disposition of the screws cannot be varied.
S 20 The objective of the present invention is to provide a mechanically improved securing of the osteosynthesis plate by having freedom of choice for the direction of the screws so that, in this way, adequate anchoring and immobilisation of the neck of the femur can be achieved with the simplest means, and the greater trochanter is immobilised by means of the osteosynthesis plate.
According to one aspect of the present invention there is provided an osteosynthesis aid for use in a case of bone fracture, comprising an intramedullary nail and an osteosynthesis plate, said intramedullary nail having at an end thereof means for connection to said osteosynthesis plate, said osteosynthesis plate having plural spaced bores 'for receiving bone screws, and said bores being arranged in s Iowned -4 2 two longitudinal rows, said rows being transversely spaced, wherein the transverse spacing of rows is greater than the diameter of said intramedullary nail.
According to another aspect of the present invention there is provided an osteosynthesis aid for use in the case of a pertrochanteric fracture of a femur, comprising an intramedullary nail for insertion into the proximal region of a femur and an osteosynthesis plate having bores for receiving short and long bone screws, said intramedullary nail being of sufficient length to extend into a greater trochanter and including at an upper end connection means for establishing a connection to said osteosynthesis plate, said bores in said osteosynthesis plate being transversely spaced in a manner such that bone screws inserted therein can bypass both sides of said intramedullary nail; and said screws comprising relatively short bone-screws for fixation of said osteosynthesis plate to a femur, and relatively long bone-screws to lead from an outer side of said osteosynthesis pi ;e in traverse of a fracture and into a femoral neck.
According to another aspect of the present invention there is provided an osteosynthesis aid for use in the case of a fracture of a humerus, comprising an intramedullary nail ior insertion into the proximal end of a humerus and an osteosynthesis plate having plural spaced bores for receiving bone screws, said nail being of sufficient length 4 to extend both sides of said fracture and including at an externally accessible proximal end connection means for attachment to the osteosynthesis plate, the bores of said plate being transversely spaced so that bone screws inserted therein can bypass on both sides of said nail.
Stated in other words, it is proposed that, in the case of a transtrochanteric femoral neck fracture, an intramedullary nail is inserted in the proximal region of i, I b 4

i i i i i I i 11~1 I i r I r i 2A the femur, which extends right up into the greater trochanter and, on its upper end, there is a connection device designed, by way of example, as a screw-threaded fitting for holding the osteosynthesis plate. The 5 osteosynthesis plate is furnished wich a plurality of drilled holes for receiving bone screws, in which case the transverse distance between the holes is greater than the diameter of the initramedullary nail, so that the direction of the screws is not predetermined by corresponding drilled holes in the intramedullary nail. In this manner the holes can be arranged in such a way that the screws inserted through them pass by both sides of the intramedullary nail and lead to the head of the femur, in which case the direction of the screws may be freely selected.
15 This ensures that secure positioning of the neck of the femur and of the head of the femur is achieved, in which case the secure positioning is substantially attainable with the conventional instrumentarium.
Furthermore, it is proposed in accordance with an embodiment of the invention applicable in the case of an upper arm humerus fracture in the region of the head of the humerus, that an osteosynthesis plate is provided which may be rigidly attached to an intramedullary nail inserted into the humerus, in which case the osteosynthesis plate is additionally furnished with drilled holes to receive screws, which are disposed in such a way that these screws pass by both sides of the intramedullary nail.

p ~-p -3- Examples of embodiment will now be dealt with in greater detail with reference to the accompanying drawings, in which: Fig. 1 is an elevation, partly in section, of the proximal end of a femur, Fig. 2 is a cross-section along line 2 2 in Fig. 1, and Fig. 3 is the upper end of a humerus with an osteosynthesis aid.
In Fig. 1, the letter A designates the shaft of the femur, letter B designates the neck of the femur and letter C designates the head of the femur. An intramedullary nail 3 is inserted into the proximal end of the shaft A of the femur, this nail having at its upper end a screw-threaded connection 1, by means of which an osteosynthesis plate 4 may be connected to the intramedullary nail 3. The osteosynthesis plate 4 is furnished with a plurality of drilled holes 5 through which long bone screws 2 and short bone screws may be inserted. The short bone screws 6 hold the osteosynthesis plate firmly against the shaft A of the femur, whereas the long bone screws 2 extend up into the head of the femur and therefore rigidly attach this region to the upper end of the femur.

As dearly shown in Fig. 1 and also in Fig. 2, the drilled holes 5 for receiving the screws 2 and 6 are arranged in such a manner that the inserted screws pass by both sides of the inserted intramedullary nail and screws 2 extend up into the head of the femur. This makes it un-necessary to have a direct connection to the intramedullary nail, so that expensive nail constructions can be avoided.
In the embodiment depicted in Fig. 3, The upper end of a humerus is depicted with the letter D designating the head of the humerus and the letter E designating the shaft of the humerus. An osteosynthesis plate 4a is rigidly attached to the head of the humerus, this plate being furnished with drilled holes 5a to receive the fastening screws. Furthermore, an intramedullary nail 3a may be rigidly connected to this osteosynthesis plate 4a, so that this intramedullary nail is retained firmly in position by the osteosynthesis plate affixed to the head of the humerus.

Claims (6)

1. An osteosynthesis aid for use in a case of iI bone fracture, comprising an intramedullary nail and an osteosynthesis plate, said intramedullary nail having at an end thereof means for connection to said osteosynthesis plate, said osteosynthesis plate having plural spaced bores for receiving bone screws, and said bores being arranged in i two longitudinal rows, said rows being transversely spaced, wherein the transverse spacing of rows is greater than the diameter of said intramedullary nail. ,00

3. An osteosynthesis aid according to claim 1 or j 2, wherein said osteosynthesis pl te is capable of being deformed for adaptation to the contour of a given bone.
4. An osteosynthesis aid for use in the case of a fracture of a humerus, comprising an intramedullary nail for insertion into the proximal end of a humerus and an osteosynthesis plate having plural spaced bores for t J I
