Description
より特別には外科用の改良された被吸収性ねじ及び被覆
ねじに関する。
多くの外科的手法がある。ある外科的な用途は、復位し
た前部十字靭帯の端部を取り付けるために定位置に骨栓
を固定するための骨隧道内への干渉ねじの挿入を含む。
前部十字靭帯再構成法及び干渉ねじは、例えば米国特許
第5062843号、第4950270号及び第4927421号並びに次の

さが大きくかつねじの深さが小さいポリマー干渉ねじを
特徴とする。ねじの厚さの増加は被吸収性干渉ねじの総
合剛性及び強度を強化する。ねじの食いつきと保持力の
増加並びに壁厚の増加のために、ねじの手元側領域は内
径及び/又は外径が手元側方向に徐々に増加する。ねじ
の末端領域はねじ山に鋭いピークのある1回又は複数回
の巻回を有する螺旋状ねじとすることができる。あるい
は、末端領域は全くねじがなく平滑な円錐台部分であ
り、手元側領域はねじが設けられ、その直後で直径が増
大し始める。
被覆は、ねじの摩擦係数を高くし又は低くするように、
好ましくは骨の成長を促進するために生物分解するよう
に、あるいは薬剤又は成長因子のような物質の運搬者と
して作用するように選定される。
付属図面の以下の説明より分かるであろう。図面におい
て、 図1は通常の金属の5.5mm干渉ねじの側面図であり; 図2は本発明による新規な5.5mm被吸収性干渉ねじの
側面図であり; 図3は通常の金属の7mm干渉ねじの側面図であり; 図4は本発明による新規な7mm被吸収性干渉ねじの側
面図であり; そして 図5は本発明により被覆された新規な9mm被吸収性干
渉ねじの部分的な側方断面図である。

のが図1及び3に図示される。

動用ソケット20が手元側端部18に定められる。

残りの部分上では、ねじの厚さは通常の金属ねじのもの
より大きい。ここに図示された総てのねじについて、末
端領域DRの螺旋ねじは外径テーパー角度MAT、及び内径
テーパー角度MITを有し;ポリマーねじの末端領域は領
域DRAと呼ばれる。

の両者とも、手元側領域PRにおいて、これらの寸法を得
るように約4゜の角度で増大する。10゜までのテーパー
角度が望ましく、3゜から5゜が好ましい。

かつねじ42の手元側の残余部分に見られるような厚い山
でなくピーク又はエッジ状に尖る。

も1巻きがエッジを形成し、末端の先端部76には丸みが
付けられる。手元側領域PRの長さは2.692から12.7mm
(0.106から0.50インチ)である。その他の寸法は表I
−IIIに説明される。駆動ソケット78は、末端方向にソ
ケット底部80又は80Aにおいて終わる。

鋭いエッジを有し、細長いねじの体部97は丸みの付けら
れた末端側先端98で終わる。多角形の駆動ソケット100
はソケット底部102又は102aで終わる。
可能なデルリンのような被吸収性でないポリマー、又は
米国特許第3739773号に開示されたポリラクチクアシッ
ド(ラクチド)、ポリグリコリクアシッド(グリコリ
ド)のような生物被吸収性材料、又は米国特許第430056
5号(ローゼンシャフト他)及び第4429080号(キャゼイ
他)に開示されたコポリマーで形成することができる。
これら文献の総ては参考文献としてここに組み入れられ
る。部分的に被吸収性の移植物を形成するために被吸収
性材料と被吸収性でない材料との組合せも利用すること
ができる。ラクチドのようなポリマーはその遅い被吸収
速度、従って構造的一体性のより長期の保持のため好ま
しい。靭帯の植接用片を定位置に固定するために使用さ
れる干渉ねじについては、移植後12週間で少なくとも50
%の強度の保持が好ましい。
擦係数を有することが好ましい。受容しうるポリカプロ
レートコポリマーは85重量%のε−カプロラクトン及び
15重量%のグリコリドのランダムコポリマーである。そ
の他の適切な被覆が、参考文献としてここに組み入れら
れた米国特許第4788969号(ジャレー他)に開示され
る。被吸収性被覆は、挿入回転力の低下に加えて、薬
物、成長因子、又はその他の化合物を含むことができ
る。更に、被覆は骨の内方成長及びねじへの付着を強化
するために所望期間内で見えなくなるように選択された
被吸収特性を持つことができる。
て、本発明に従ってねじ10及び50、図1及び3、のよう
な金属ねじにもこれを設けることができる。被吸収性被
覆の上述の利点は非ポリマーの被吸収性でない外科用ね
じにも適用する。かかる被覆されたねじは、骨折の固定
に特に有用であり、また、カニューレ挿入式又は非挿入
式とすることができる。
性被覆は、ねじ主体部の吸収速度に影響する体液に対す
る障壁として作用することができる。本発明による被吸
収性被覆は、ねじ体部とは異なった組成を有し、異なっ
た摩擦係数又は吸収速度のような1種又はそれ以上の選
択された異なった特性を与えることができる。
溶液重量/体積を形成するように80ミリリットルのアセ
トンに10グラムのポリカプロレートコポリマーの溶液
(カプロラクトン85重量%、グリコリド15重量%)に浸
漬された5個のチタン合金のねじにより提供された。ね
じは総計7回、交互に浸漬され乾燥された。
れた図3に示される形状を有する7mm×25mmの被覆なし
のカニューレ非挿入式の5個の干渉ねじ、及び上述のよ
うにポリカプロレートで浸漬被覆された5個の同様なね
じの、総計10個のねじが試験された。各ねじの1個が牛
骨の顎の中央領域内に、各2個が顎の後方領域内に、そ
して各2個が顎の前方領域内に挿入された。結果は次の
表IVに与えられる。

これらは、各特徴が本発明によるその他の特徴のいずれ

すぎない。

Claims (6)
- 【請求項1】移植用片固定栓を定位置に固定するために
骨に穿孔された隧道内に挿入し得る干渉ねじであって、
末端と手元側端部とを有する細長い実質的に円柱状のね
じ体部を備え、前記手元側端部は、駆動具に係合する手
段を形成しかつ頭部部材を欠き、前記細長い体部は生体
吸収性のポリマー材料で作られ、前記細長い体部はその
外周の周りの螺旋状ねじを定めかつ体部外径と体部内径
とを有し、かつ前記細長い体部は末端方向に進むに従っ
て直径が徐々に減少しかつ前記末端において終わる末端
領域と、手元側端部に向かって直径が外方に傾斜した手
元側領域と、手元側領域より末端寄りにあって一定の直
径を有する部分とを更に定め、更に前記手元側端部に向
かって手元側方向に進みながら、(a)前記一定の直径
を有する部分の外径より大きな手元側領域外径、及び
(b)一定の直径を有する部分の内径より大きな手元側
領域内径の少なくとも一方が増加する手元側領域を定
め、更に前記螺旋状ねじは、前記ねじの少なくも1巻き
の間、手前側領域より末端寄りの部分で手元側領域のね
じ幅よりも狭いねじ幅を有しかつ前記手元側領域より末
端よりの部分のねじ幅より実質的に狭いねじ幅で前記末
端領域上を伸びる干渉ねじ。 - 【請求項2】前記細長い体部がその末端において実質的
に丸められた端末先端部で終わるとともに、生物吸収性
材料で形成されかつ更に前記細長い体部の外面の周りに
配置された生体吸収性被覆を有し、前記被覆が前記細長
い体部の摩擦係数より小さな摩擦係数を有する請求の範
囲1の干渉ねじ。 - 【請求項3】前記被覆がポリカプロレートタイプの材料
である請求の範囲2の干渉ねじ。 - 【請求項4】前記ポリカプロレートタイプの材料がε−
カブプロラクトン85重量%とグリコリド15重量%のラン
ダムコポリマーである請求の範囲3の干渉ねじ。 - 【請求項5】前記手元側領域外径が10゜までの角度で外
向きにテーパーになっている請求の範囲1の干渉ねじ。 - 【請求項6】前記手元側領域外径が約3゜から5゜まで
の角度で外向きにテーパーになっている請求の範囲1の
干渉ねじ。
