【発明が属する技術分野】
本発明は、靭帯再建術において靭帯を骨から吊り下げるためのアンカーを用いた再建用靱帯の固定装置であり、例えば整形外科分野における膝前十字靭帯再建術において脛骨側の固定として用いる。

【従来の技術】
この種の整形外科手術では、靭帯再建術における靭帯の固定方法として例えば靭帯を大腿骨から吊り下げる固定方法として図1で示すようなエンドボタンと呼ばれるアンカー及びエンドボタンと靭帯とを結ぶテープや糸、人工靭帯等の組み合わせが用いられている。このエンドボタンは米国で特許が取得されているが日本では特許出願されていない。

このエンドボタン1を用いる靭帯再建法では、膝に骨孔2、2をあけ、機能しなくなった靭帯のかわりに、骨孔2、2に通した、ループ状に形成した靭帯3と、骨孔2の外においたエンドボタン1とを、テープや糸、人工靭帯等の吊り下げ部4でつなぎ固定するものである。このような固定は主に太腿骨側で行い、脛骨側は吊り下げ部4をステープル5等で骨に固定することにより靭帯再建を行う。

上記エンドボタン1は図2に示すように盤状物部に穴が開いている構造であった。

【発明が解決しようとする課題】
上記固定方法には次に述べるような解決を必要とする課題が残されていた。

大腿骨側の方法が上記のごとくあるのに対し脛骨側はよい方法がなかった。これまでは吊り下げ部4を固定するためにステープル5を用いる時、ステープル5の骨に対する圧迫固定力が不十分で、吊り下げ部4が十分固定できず抜けて緩むことがあった。図3のごとく吊り下げ部4を骨孔外のエンドボタン1に通し結んで固定する方法も可能であるが、結び目6で緩む可能性があった。

これらの緩みは再建靭帯の緩みをもたらすため靭帯再建術の目的が無意味になる可能性を秘めているという問題があった。

そこで本発明は、上記した従来技術の課題を解決してゆるんだりほどけたりしない靭帯再建用の固定装置を提供する。

【課題を解決するための手段】
本発明による靭帯吊り下げ用アン力ーの基本的構造は、盤状物部に通した通常2本からなる吊り下げ部を結合させる時この結合部をいかにして緩まない結合にしループを作るかということに関するものであり、熱処理等により吊り下げ部をループ状に結合させるという手段を用いる。またループ状に結合させるまでに吊り下げた靭帯に一定の張力をかける方法を同時に示す。

脛骨側は吊り下げ部のループが緩まないことにより、再建靭帯の緩まない固定が脛骨側でも可能になる。

【発明の実施の形態】
以下に、本発明による靭帯吊り下げ用アンカーについて、望ましい発明の実施の形態に基づいて詳細に説明する。

第1実施例の靭帯吊り下げ用アンカーは、図4で示すように、盤状物部7の2つの穴8、8に、テープや糸、人工靭帯等の吊り下げ部9がとおしてある(図4−a)。この吊り下げ部9の両端を結び、もしくは交差させ、もしくは交差させずに重ねた部分10に、熱を加えるための装置11で熱を加えることで(図4−b)、その部分が融合しループ状部が構成できる(図4−c)。

第4実施例の靭帯吊り下げ用アンカーは、第2実施例の靭帯吊り下げ用アンカーと同じ目的であるが図7で示すように、中央に窪み15があり(図7−a)この部分が骨孔2内へ挿入できる(図7−b)。窪み15の中で、2つの穴8、8から出た吊り下げ部9を結び、もしくは交差させ、もしくは交差させずに重ね、ループを作成する(図7−c)。この時結合部16は、皮下に突出しないため、痛みを誘発しにくい。

第6実施例の靭帯吊り下げ用アンカーは図4で述べた如く熱を加えることで、テープや糸、人工靭帯等の吊り下げ部9を融合させるのではなく、図9で示すように、吊り下げ部9を結び、もしくは交差させ、重ね、もしくは交差させずに重ねた後、重ねた部分10を外から巻くもの17(図9−a)や、包み込むもの18(図9−b)や、貫くもの19(図9−c)で固定することにより、結合部16を作ることでループ状部を構成することができるような、締結具の例を示す。貫くもの19のような締結具は中央が折りたためる両端に歯20があり結合部16でこの締結具を折りたたむことで吊り下げ部9を結合させループ状部を作ることができてよい。テープや糸、人工靭帯等の吊り下げ部9は結合部16がずれないようでこぼこ部21があってもよい(図9−d)。

なお、本発明は上記した実施例に限定されるものではなく要旨の範囲内において各種の変形を採り得るものである。

【発明の効果】
上記した説明により明らかな様に、本発明の靭帯再建用の固定装置によれば、図1で示したステープルの様な緩み得る固定でなく、また図3で示したエンドボタンの様にループを作った後、結び目が緩むことがないため、強固な緩まない脛骨側の、ひいては靭帯の緩みやずれのない固定を提供できる。
