【発明の属する技術分野】
本発明は、自力で起き上がることのできなくなった病人や老人等の被介護者の起き上がり動作や、排便、更には車椅子等への移動が容易に行える介護用ベッドに関するものである。

【従来の技術】
従来のこの種の介護用ベッドには、これに寝ている被介護者が排便する際に使用する便器を備えたり、また便器に加えてこれに被介護者を座らせるために利用するリクライニング機構等を備えたもの(特開平11−151271号等)は比較的数多く提案されている。

しかしいずれも用便時に、便器をスムーズに使用可能な状態に状態変化させるのが厄介であったり、ベッドに寝ている被介護者を該便器上に移動させたり、座らせたりするのがなかなか厄介であったり、更にはその姿勢を良好に保持させることができず、それ故、被介護者が良好な排便動作を行いにくかったりと、それぞれがいずれかの問題を抱えている。

また該介護用ベッドは、これに寝ている被介護者を様々な理由から車椅子等に乗せたい場合がしばしばあり、それなりの機構が考えられているようであるが、これを簡易かつ良好に行うための満足すべき手段は提供されていない。

【発明が解決しようとする課題】
本発明は、以上のような従来の介護用ベッドの問題点を解決し、便器を備えるとともに、必要な際には、該便器を使用可能な状態にスピーディかつ簡単に移行させることができ、またその際に、寝ていた被介護者を介護者に無用の負担をかけずに起こして、該便器上に移動させ得る介護用ベッドを提供することを解決の課題とする。また被介護者がベッド上から車椅子等に簡易に移動することのできる手段をも備えた介護用ベッドを提供することを解決の課題とする。

【課題を解決するための手段】
本発明の1は、ベッド本体の上部に配した分割マットレスであって、就寝する人の頭部側に配する背部対応部、腰部対応部及び脚部対応部からなる分割マットレスと、
前記背部対応部をその内端側を支点にして起伏させる起伏駆動機構と、
前記背部対応部及び前記腰部対応部を、その定位置と移動によって該腰部対応部の後端が前記ベッド本体の後端に到達することとなる位置との間で往復動させるための往復駆動機構と、
前記脚部対応部を、定位置に位置する背部対応部及び腰部対応部の下方に進退自在に進入させるための進退駆動機構と、
前記脚部対応部の下方に配した便器と、
前記ベッド本体の後端に前記分割マットレスの脚部対応部側を向け得るように配した一対の棒状支持材を備えた脇の下支持部であって、該棒状支持材で両脇の下を支持された被介護者が前記ベッド本体の後端外方で立ち上がり得るように、該後端外方の斜め上方に向かって該棒状支持材を昇降自在に移動し得るように構成した脇の下支持部と、

【発明の実施の形態】
本発明は、基本的に、背部対応部、腰部対応部及び脚部対応部からなる分割マットレスと、上記背部対応部を起伏させる起伏駆動機構と、上記背部対応部及び腰部対応部をベッド本体の後端(就寝する人の脚部側となる端部)との間で往復動させるための往復駆動機構と、前記脚部対応部を背部対応部及び腰部対応部の下方に進退自在に進入させるための進退駆動機構と、前記脚部対応部の下方に配した便器と、被介護者を便器に座らせる際又は車椅子に乗せる際に使用する脇の下支持部とで構成した介護用ベッドである。

前記分割マットレスの内、背部対応部及び腰部対応部は、一般のマットレスとしての機能を有するとともに、相互を背部対応部が起伏自在であるようにヒンジ等で結合する。上記背部対応部及び腰部対応部は、それぞれその両側部及びベッド本体の対応する部位に前後方向への移動手段を構成する。例えば、それぞれの両側部にスライド部材を設け、これらをベッド本体の対応部位に配した案内レール上にスライド自在に載置する構成とすることができる。あるいはそれぞれの両側部に回動自在なローラ部材を取り付け、これらをベッド本体の対応する部位に配した案内レール上に転動自在に載置することができる。

前記分割マットレスの内、脚部対応部は、その前端及び後端に配した回転自在なローラ部材と、これらに掛け渡した無端ベルト部材と、該無端ベルト部材の上方側の直下に配した滑面部材とで構成するのが適当である。このように構成することにより、後述するように、前記背部対応部を起こして、ベッド上に寝ている被介護者を起こし、この状態で該背部対応部及び腰部対応部をベッド本体の脚部端側にスライド移動させる際に、被介護者の脚部が前記脚部対応部上で滞ることなく、スムーズにベッド本体の脚部側端を越えてその後方まで移動することができることとなるからである。

前記脚部対応部にも、前記背部対応部及び腰部対応部と同様に、その両側部及びベッド本体の対応する部位に前後方向への移動手段を構成する。例えば、その両側部にスライド部材を設け、これらをベッド本体の対応部位に配した案内レール上にスライド自在に載置する構成とすることができる。あるいはその両側部に回動自在な転動ローラを取り付け、これらをベッド本体の対応する部位に配した案内レール上に転動自在に載置することができる。以上の案内レールは、いずれにしても前記脚部対応部が、前記背部対応部及び腰部対応部の直下に進退自在に移動する必要があるので、その高さ位置に配すべきである。

なお前記脚部対応部は、前記背部対応部及び腰部対応部より、少なくともその厚み分だけは低い位置に位置することとなるので、被介護者が単に寝ている場合には、その凹みが不都合になる。それ故、この凹みを補う補助マット等をその上に配することとするのが適当である。

前記背部対応部を起伏させる起伏駆動機構は、該背部対応部を起伏させることができる機構であれば特定のそれに限定されない。例えば、主たる装置として油圧又は空気圧シリンダを採用し、該シリンダ本体の下端を上記腰部対応部に固設した支持フレームの一部に起伏自在にピン結合し、そのロッドの先端を前記背部対応部の裏面にピン結合する等によって構成することができる。

なお前記起伏駆動機構として、前記背部対応部の下方に、上記のように、空気圧シリンダを用いた機構を構成する場合、若しくは、同様に、該背部対応部の下方に位置する他の構成の機構を配する場合は、いずれにしても、該背部対応部の下方には前記脚部対応部が進退自在に進入するので干渉を生じないように相互の関係を調整する必要がある。

例えば、前記背部対応部の下方に構成すべき空気圧シリンダ等を用いた起伏駆動機構を、その下部両側に設置し、前記脚部対応部を、該背部対応部の下方に進入しても、それらの起伏駆動機構に接触することのないような幅狭に構成する等である。

前記往復駆動機構も、前記背部対応部及び腰部対応部を往復動作駆動することができれば、特定の構成のそれに限定されない。この往復駆動機構も油圧又は空気圧シリンダの伸縮動作を利用して該背部対応部等を進退駆動するように構成することができる。または電動モータを利用し、これを前記腰部対応部の下部等に取り付け、該電動モータの駆動軸に減速機等を介して結合したピニオンを正逆回転駆動し、これを、前記背部対応部等が往復動すべき範囲内に、その方向に合わせてラックを配するとともに、該ラックに噛み合わせて、これに沿って往復動するように構成することができる。勿論その他の構成を採用することも自由である。しかし前記脚部対応部の背部対応部等の下方への進入を妨げない位置関係で設置する必要はある。

前記進退駆動機構は、前記脚部対応部を前記所定の範囲内で進退移動させることができるものであれば、特定の構成のそれに限定されない。この進退駆動機構も前記往復駆動機構と同様に構成することができる。なお位置関係は、前記のように、前記背部対応部及び前記腰部対応部の直下の高さの位置である。

前記便器は、前記脚部対応部の下方に配置するものであり、その下部に該便器に排便された便を受け取る便槽をその下部に備えたものである必要がある。臭気等を周囲に撒き散らさないように、用便時にはその内部を陰圧に保持する手段等を備えていることが好ましい。また便器内を用便後に洗浄できるように、水洗手段を備えていることが好ましい。

前記脇の下支持部は、分割マットレスの背部対応部を起こし、これを腰部対応部とともにベッド本体の後端側に移動させた際に、該背部対応部によって起こされ、ベッド本体の後端側に移動した被介護者の脇の下にその棒状支持材をそれぞれ差し込んで、該被介護者を支持しつつ立ち上がらせ、分割マットレスの背部対応部及び腰部対応部を定位置に復帰させ、かつ脚部対応部をそれらの下部に移動させて、その下部の便器を露出させるとともに、該脇の下支持部の棒状支持材を下降させて被介護者を該便器に腰掛けさせるように使用でき、またその逆の動作に使用できるものである必要がある。

したがって具体的には、前記脇の下支持部は、その一対の棒状支持材を前記ベッド本体の後端に、前記脚部対応部側を向け得るように配し、該一対の棒状支持材によって両脇の下を支持された被介護者が前記ベッド本体の後端外方で立ち上がり得るように、該棒状支持材を、ベッド本体の後方の斜め上方に向かって昇降自在に移動し得るように構成したものである。

更に具体的には、前記脇の下支持部は、前記ベッド本体の後端付近の両側からそれぞれ後方に傾いた状態で昇降伸縮自在に立ち上げた一対の支持脚と、該両支持脚の上端間をつなぐ連結部材と、該連結部材の中央部に人の肩幅を越える程度の間隔で隙間をあけ、かつその隙間の両端にそれぞれ回動自在に軸着した一対の棒状支持材であって、不使用時にはその全体を該連結部材上に載置状態に位置させ、使用時にはベッド本体の上方で相互が平行状態になるように回動させ得るように構成した棒状支持材とからなるものである。

また以上のような脇の下支持部は、その一対の支持脚のそれぞれと前記連結部材の両端との各結合を後者が前者に対して水平方向に回動自在かつ着脱自在であるように構成するとともに、前記連結部材の両端付近からベッド本体の後端外方に延長した補助脚を備えさせ、かつ該補助脚の下端には自在車を取り付ける構成を採用したものである。

したがって本発明の介護用ベッドによれば、次のように、自力で起き上がることができない老人や重病人等の被介護者をこれに寝せると、上半身を起こす場合、用便をさせる場合、あるいはベッドから降ろして車椅子等に移動させる場合に、容易にそのような動作を補助することができて好都合である。

本発明の介護用ベッドに寝ている被介護者の上半身を起こす場合は、前記起伏駆動機構を動作させ、前記分割マットレスの内、背部対応部を起立動作させれば良い。これによって被介護者は背中から押し上げられて、上半身を徐々に起こすことになる。勿論、この場合、必要に応じて、身体にかけてある毛布や布団の内、上半身にかけてある部分を足下の方に折り返し、あるいは全体を取り除いておくこととする。

用便をするために上半身を起こした場合は、掛け布団等を取り除き、かつ前記分割マットレスの内、脚部対応部上に載せてあった補助マットを取り除いた上で、前記往復駆動機構を動作させ、前記分割マットレスの内の背部対応部及び腰部対応部をベッド本体の後端側に移動させる。こうして被介護者をベッド本体の後端側に移動させ、ついにはその両脚を該ベッド本体の後端から垂下させるようにする。なおこのような被介護者の移動の際に、その両脚は、前記脚部対応部上をベッド本体の後端側に移動することとなるが、前記のように、これを回転自在なローラ部材と、これらに掛け渡した無端ベルト部材と、該無端ベルト部材の上方側の直下に配した滑面部材とで構成したため、被介護者の両脚の移動に伴って該ベルト部材の上方側が同方向に移動するので、無理な摩擦等が生ぜず、楽に移動させることができ、好都合である。

こうして被介護者がベッド本体の後端まで移動し、その両脚を該部位から外側に垂下させるような状態になる時点で、あるいはその前に、前記脇の下支持部の一対の棒状支持材を人の脇の下の幅で平行にベッド本体の前方を向けた状態に回動させておけば、移動されてきた被介護者の脇の下にそれぞれを挿入し、該被介護者の上半身を起こした状態をこれらによって支持することができる。背中支持用のベルトを用意しておけば、これを被介護者の背中に回し、該ベルトの両端をそれぞれ前記一対の棒状支持材の端部に結合する事で、被介護者をより安全性高く支持することができる。

以上の状態から、前記脇の下支持部の棒状支持材をベッド本体の後方に向かって斜め上方に上昇させると、被介護者は脇の下で支えられた状態で、その上半身が徐々に後方に移動させられながらベッド本体の後端外方に移動して立ち上がることとなる。

被介護者をこのように脇の下で支持して立ち上がった状態を保持する間に、前記往復駆動機構を動作させ、前記分割マットレスの背部対応部及び腰部対応部を定位置まで復帰させ、更に同時に又は順次、前記進退駆動機構を動作させて、前記分割マットレスの脚部対応部を、定位置に復帰した背部対応部及び腰部対応部の下方に移動進入させる。

こうして前記脚部対応部が定位置から前記背部対応部等の下方に移動してしまうと、該脚部対応部に被覆されていた便器がその下から現れる。ここで前記脇の下支持部の棒状支持材を下降動作させると、該棒状支持材は、前記したのとは逆に、ベッド本体の前方に向かって斜めに下降する動作になるので、これによって支持されている被介護者はその下降とともに下降し、ついには前記便器に腰掛ける状態になる。

したがって該脇の下支持部の棒状支持材の下降動作を開始させる前に、被介護者が立っている状態で、必要な着衣を脱がせる等の処置をしておけば、該棒状支持材を必要なだけ下降動作させることにより、被介護者を、上記のように、用便可能な状態で前記便器に腰掛けさせることができることとなる。そして被介護者は、この状態で、該棒状支持材及び背後のベルトで安全に保護されつつ排便動作を行うことができる。

排便の終了後には、該棒状支持材を上昇動作させ、被介護者を立ち上がらせて、必要な処置を取り、かつ着衣を着せ、これと前後して、前記進退駆動機構を動作させて、前記分割マットレスの脚部対応部を、定位置、即ち、便器の上に復帰させる。なお便器の水洗手段を適当な時点で動作させて洗浄すべきなのは云うまでもない。

この後、若しくは、前記脚部対応部を定位置に復帰させるのと同時に、前記往復駆動機構を動作させ、前記分割マットレスの背部対応部及び腰部対応部をベッド本体の後端側に移動させ、次いで、また前記脇の下支持部の棒状支持材を下降動作させて、被介護者を、前記脚部対応部上に位置する腰部対応部上に腰掛けさせる。このとき、前記背部対応部を伏状態に復帰させてあった場合は、前記起伏駆動機構を動作させて、再度起立させ、また起立状態のままに維持しておいた場合は、そのまま起立状態を維持しながら、被介護者の背中を支持していた脇の下支持部のベルトを取り外し、該背部対応部に寄りかからせるようにする。

この状態で、また前記往復駆動機構を動作させて前記背部対応部及び腰部対応部を定位置に復帰させ、次いで前記起伏駆動機構を伏動作させて、該背部対応部を伏状態に戻し、その後、前記脚部対応部に補助マットを載せ、加えて、先に取り除いた掛け布団等を被介護者の上に載せれば、寝ている状態から排便、そして寝ている状態に復帰する動作の全てが完了する。

該被介護者を、以上のようにベッド本体の後方に移動させ、かつ上記のように、ベッド本体の長さ方向に直交する向きに背中を向けさせると、該被介護者の背後に車椅子を移動させることが容易になり、かつ背後に移動させた車椅子に被介護者は単に腰を下ろすだけで、容易に着座することができることとなる。

前記分割マットレス2の内、背部対応部2a及び腰部対応部2bは、それぞれ表面側は通常のマットレスとしての弾力性その他の機能を備えるとともに、裏面側にはこれらを板状に保持する補強材を備えたもので、相互間は、図1及び図2に示すように、該背部対応部2aが該腰部対応部2bに起伏自在になるように一対のヒンジh、hで結合してある。

前記起伏駆動機構3は、既述のように、前記分割マットレス2の背部対応部2aを起伏させる機構であるが、これは、図2及び図7に示すように、前記腰部対応部2bの裏面の両側(前記脚部対応部2cの両側より外側にそれぞれ固設した支持フレーム3a、3aと、一対の空気圧シリンダ3b、3bと、図示しないコンプレッサその他からなる圧縮空気の発生装置とで構成するものであり、上記空気圧シリンダ3b、3bは、それぞれその下端を上記支持フレーム3a、3aの前端に回動自在にピン結合し、各ロッドの先端を前記背部対応部2aの裏面両側に回動自在にピン結合したものである。
