Description
本発明は、骨セメントを伴うことなく患者の骨中に挿入するため或いは患者の骨に対して取り付けるための任意の取付部を有する任意の種類の人工インプラントに関する。したがって、本発明は、例えば、大腿プロテーゼ、人工膝関節の脛骨部品、または、股関節臼蓋に対して適用可能である。
用語「骨上成長」は、本明細書では、その上で骨が成長できる表面、例えば粗面を示すために使用される。用語「骨内成長」は、本明細書では、その内部で骨が内側に向けて成長できる表面、例えば多孔質面を示すために使用される。
骨セメントを伴わずに使用するために、粗い外面を有し得る合成プラスチック材料から形成されるシースを使用して大腿プロテーゼのステム上にわたって骨上成長を促すことは、例えばＵＳ５，６６５，１２１から知られている。また、ステムを取り囲むがシリコンまたはブチルゴムなどのエラストマー材料の層によってステムから分離されるステンレススチールまたはチタンスチールから形成される外側シースを大腿プロテーゼに設けることは、ＵＳ４，６５０，４８９からも知られている。金属シースの内面およびステムの外面は、エラストマー材料を所定の位置に保持し、且つステムと外側金属シースとの間の移動を防止するようになっている。エラストマー材料の弾力に起因する衝撃下でステムが僅かに移動できるように、ステムの先端は、エラストマー材料の下側の空気／ガスで満たされて閉じられたキャビティ内に位置されているが、ステムは挿入後にシース内で下側へ戻ることができるようにはなっていない。
本発明は、金属製の外面を配置することによりシースを基端荷重できるとともに、シースが骨内成長を促せるようにし、また、シースの先端が骨上成長を促し或いは非常に僅かな骨内成長だけを促し或いは骨内成長を全く促さないという点で、先の文献において言及した構造を越える利点を有する構造を提供することを意図している。この構成を用いると、スリーブの先端における荷重が基端荷重に対して減少され、望ましいことが分かった。
本発明によれば、大腿プロテーゼは、ステムの形態を成し、骨セメントを伴うことなく患者の骨中に挿入するため或いは患者の骨に対して取り付けるための取付部を有し、前記ステムの外面は、滑らかであるととともに、前記ステムの基端から先端へ移動できるようにテーパ状に形成され、合成樹脂材料から形成される第１の層がステムに対して付加され、前記ステムは取付け後の第１の層に関し、ステムが下方へ移動することを可能にする適切な平滑面を有し、金属材料からなる第２の層が前記第１の層に固定され、前記第２の層の第１の基端部は、骨内成長または骨上成長を促すために多孔質であり或いは粗面化されており、前記第２の層の第２の先端部は、前記第１の基端部に対して孔が少なく、殆ど孔が無く或いは殆ど粗面化されておらず或いは滑らかである。
このタイプの構造によれば、望ましい基端荷重能力を得ることができる。
したがって、金属層の外面は、例えば、骨内成長が必要とされる基端部の部位に通気孔を有し、骨上成長が必要とされる金属層の外側先端面の部位に粗面を有し、或いは、この部分は比較的滑らかな場合がある。
幾つかの構成において、必要とされる基端固定は、基端部上の粗面と、先端部上の滑らかな或いは殆ど粗面化されていない面とによって達成できる。
第１および第２の層は、その場で取付部に対して付加することができ、例えば、好ましくはプラスチックシースを取付部上の所定位置に流し込んで取付部と共に使用できるようにし、また、これに対して、一時的に、先端空間を形成するための機構を取り付けることができる。
或いは、第１および第２の層を別個のシースとして予め形成し、取付部に対して取り付けることもできる。この場合も先と同様に、プラスチックの内側層を意図される大腿インプラント上に流し込むことができ、或いは、この内側層を外側金属層内へ流し込んで成形することもできる。
また、シースは、意図される大腿インプラントと優先的に適合しないように形成することができ、例えば、シースは、既存のインプラントと共に使用できる標準的な寸法を有していてもよい。
第２の金属材料層は、チタン、チタン合金、または、骨と直接に接触する任意の他の適切に生体適合する金属から形成することが可能である。
金属は、予備成形形状の形態を成すことができ、或いは、金属スパッタリングによって形成することが可能であり、或いは、例えばレーザ溶融プロセスによって形成することが可能である。
層溶融プロセスは、例えば、「Ｌａｓｅｒ−Ｐｒｏｄｕｃｅｄ Ｐｏｒｏｕｓ Ｓｕｒｆａｃｅ」と題された米国特許出願第２００４０１９１１０６号；「Ｇｒａｄｉｅｎｔ Ｐｏｒｏｕｓ Ｉｍｐｌａｎｔ」と題された第２００６０１４７３３２号；「Ｌａｓｅｒ−Ｐｒｏｄｕｃｅｄ Ｐｏｒｏｕｓ Ｓｕｒｆａｃｅ」と題された１１／２９５，００８号；「Ｌａｓｅｒ−Ｐｒｏｄｕｃｅｄ Ｉｍｐｌａｎｔｓ」と題された６０／７５５，２６０号に記載されており、これらの開示内容は参照により本明細書に組み入れられる。米国特許出願第２００４０１９１１０６号に記載されるように、選択レーザ溶融または焼結プロセスを使用して金属構造体が構成されてもよく、これにより、層状化プロセスによって層内で構造体が成長されることになる。別の方法では、米国特許出願第２００４０１９１１６号に記載される交互プロセスを使用して金属構造体が形成されてもよい。この場合、中間部がベースまたは基板としての機能を果たし、このベース上に多層様式で高分子係合部および骨内成長部が形成されている。参照によりその開示内容が本明細書に組み入れられる「Ｐｏｒｏｕｓ Ｍｅｔａｌｌｉｃ Ｓｃａｆｆｏｌｄ ｆｏｒ Ｔｉｓｓｕｅ」と題された米国特許出願公開第２００３０１５３９８１号に開示されるような、金属格子を構成するための更なる技術、および、参照によりその開示内容が本明細書に組み入れられる特許出願公開第２００６０２２８２４７号および第２００６０００２８１０号に開示されるような、当業者に知られる更なる方法が使用されてもよい。
合成樹脂材料は、ポリメチルメタクリレート（ＰＭＭＡ）であることが好ましい。
例えば、第２の層の内面上に粗面を設けることによって、第１の層と第２の層との間に係合または結合が存在することが好ましい。
第１および第２の層の先端をカップとして形成することが可能であり、その内面は、最初に所定位置に配置される際に先端空間を形成すべく取付部の先端から離間され、これにより、取り付け後における第１の層と取付部との間のその後の移動が許容されるようになっている。この移動によって、ステムは、埋め込まれた装置の最初の重量負荷後にその自然の位置へ戻ることができる。
２つの層によって形成される壁の厚さは、１ｍｍ〜３ｍｍであってもよく、好ましくは１．８ｍｍ〜２．５ｍｍである。
必要に応じて、金属の第２の層の内面は、合成樹脂の第１の層の外面に対する接着を高めるために、粗面化することが可能である。好ましくは、この接着は、ＰＭＭＡ骨セメントの付加を伴って行なうことが可能である。
また、本発明は、前述した取付部を有する人工インプラントを形成する方法も含み、本方法は、ステム部を液体材料槽中に浸漬することにより或いは取付部の外面に対して材料を吹き付けることにより合成樹脂材料からなる第１の層を取り付け、その後、合成樹脂材料層上に金属をバタリングすることにより或いは選択レーザ焼結プロセスを使用することにより金属の第２の層を付加することを含んでいる。
他の方法では、別個に形成され且つ先端−基端方向で取付部上にスライドされるシースとして第１および第２の層が設けられる。
これは、２つの層を予め形成して付加的なＰＭＭＡ骨セメントを用いて互いに接着することにより行なうことが可能である。
本発明は様々な方式で行なうことができるが、ここでは、添付図面を参照して一例として１つの実施形態について説明する。
図面に示されるように、本発明は、ステム２の形態を成す骨セメントを伴うことなく患者の骨中に挿入するため或いは患者の骨に対して取り付けるための取付部を有する大腿プロテーゼ１の形態を成す人工インプラントに適用されている。ステム２の外面は、滑らかであるとともに、ステムを基端から先端へと移動させることにより研磨されてテーパ状に形成されていてもよい。ステム２の外面は、滑らかであるとともに、この例ではポリメチルメタクリレートである合成樹脂材料により形成される第１の層３を有している。
第１の層３上にわたって金属材料から形成される第２の層４が付加され、第２の層４の基端部５は多孔質である。第２の金属層４の先端部６（大腿の骨幹内）は、基端部（大腿の骨端および骨幹端内）５に対して孔が少なく或いは殆ど孔が無い。
この構造を用いると、多孔質である第２の層４の基端部５が骨内成長をもたらし、先端部６は、それを粗面として形成することにより孔を少なくし或いは殆ど孔を無くして骨上成長を許容することが可能である。したがって、ステムが基端荷重される基端部において最大の骨結合がもたらされることになる。先端固定が不要な領域では、先端部６の外面を比較的滑らかにすることができる。
或いは、第２の層４の基端部５は、骨上成長を許容するために多孔質ではなく粗くすることが可能であり、また、先端部６は、同じ効果をもたらすためにあまり粗くせず或いはほぼ滑らかにすることが可能である。
第２の層４は、チタン、チタン合金、または、骨と直接に接触する任意の他の生体適合性金属から形成することが可能である。
第１の層３および第２の層４の先端７は、カップ８として形成されており、その内面は、最初に所定位置に配置される際に空間１０を形成すべく、ステム２の先端９から離間されている。この空間１０は、取り付け後における第１の層３とステムとの間のその後の移動を許容するものである。
層３および４によって形成される壁の厚さは、１ｍｍ〜３ｍｍであってもよく、好ましくは１．８ｍｍ〜２．５ｍｍである。
第１の層３および第２の層４は、その場でステム２に対して付加することが可能であり、例えば、プラスチックの第１の層をステム上の所定位置に流し込んでステムと共に使用できるようにし、このとき、これに対して、一時的に、先端空間を形成するための機構を取り付けることが可能となっている。
或いは、第１および第２の層を別個のシースとして予め形成し、ステムに対して取り付けることも可能である。この場合も先と同様に、プラスチックの内側層３をステム上に流し込むことが可能であり、或いは、この内側層を外側の金属製の第２の層４内へ流し込んで成形することも可能である。
このタイプのシースは、意図される大腿インプラントと優先的に適合しないように形成することが可能であり、例えば、シースは、既存のインプラントと共に使用できる標準的な寸法を有していてもよい。
製造中、第１の層３は、浸漬プロセスまたは吹き付けプロセスによってステム２に対して付加することが可能であり、また、第２の金属層４は、例えばスパッタリング或いは任意の他の層状化プロセスによって付加することが可能であり、或いは、レーザ溶融プロセスによって形成することが可能である。そのようなことは、米国公報第２００４０１９１１０６号および第２００６０１４７３３２号に開示されている。
これによって、ステムは、その表面が適切な平滑面を有していれば、取り付け後に依然として下方へ移動することが可能となる。
或いは、第１の層３および第２の層４は、適切な成形体上で、例えばそれが適合されるようになっているステム上で形成することにより、シースとして予め形成することが可能である。
金属層４の基端部が多孔質である場合、この基端部は、骨内成長を許容して、骨中への強固な固定を可能にし、また、中実な或いは孔が少ない先端部６は、骨上成長を許容するとともに、取り付け時にシースと周囲の骨との間の締まりばめとして取り付けられている。
必要に応じて、第２の金属層４の内面は、合成の第１の層３の外面に対する接着を高めるために粗くすることが可能である。この接着は、ＰＭＭＡ骨セメント（図示せず）の付加を伴って行なうことができる。
製造時、図示の大腿プロテーゼは、首部１２と、周知のタイプのプロテーゼヘッドベアリングボール（図示せず）を受けるためのテーパ状の差込み部１３とを有している。
前述した例では、本発明が大腿プロテーゼに適用されているが、本発明は、骨セメントを用いることなく患者の骨中に挿入するため或いは患者の骨に対して取り付けるための取付部を有する任意の他のプロテーゼにも同様に適用することができる。
必要に応じて、骨内成長または骨上成長を促すために、ヒドロキシアパタイトなどの生体活性材料からなる層（図示せず）を第２の金属材料層４の外面に対して付加することが可能である。
Claims (13)
- ステムの形態を成し、骨セメントを伴うことなく患者の骨中に挿入するため或いは患者の骨に対して取り付けるための取付部を有する大腿プロテーゼであって、
前記ステムの外面は、滑らかであるととともに前記ステムの基端から先端へ移動できるようにテーパ状に形成され、合成樹脂材料から形成される第１の層がステムに対して付加され、前記ステムは取付け後の第１の層に関し、ステムが下方へ移動することを可能にする適切な平滑面を有し、金属材料からなる第２の層が前記第１の層に固定され、
前記第２の層の第１の基端部は、骨内成長または骨上成長を促すために多孔質であり或いは粗面化されており、前記第２の層の第２の先端部は、前記第１の基端部に対して孔が少なく、殆ど孔が無く或いは殆ど粗面化されておらず或いは滑らかである、大腿プロテーゼ。 - 第１および第２の層は、前記取付部に対して取り付けるための別個のシースとして予め形成されている、請求項１に記載の大腿プロテーゼ。
- 金属層は、チタン、チタン合金、または、任意の他の適切に適合する生体適合性金属である、請求項１または請求項２に記載の大腿プロテーゼ。
- 金属層が予備成形形状の形態を成している、請求項１〜請求項３のいずれか一項に記載の大腿プロテーゼ。
- 金属層が金属スパッタリングによって形成されている、請求項１〜請求項４のいずれか一項に記載の大腿プロテーゼ。
- 金属層がレーザ溶融プロセスによって形成されている、請求項５に記載の大腿プロテーゼ。
- 合成樹脂材料がポリメチルメタクリレートである、請求項１〜請求項６のいずれか一項に記載の大腿プロテーゼ。
- 第１および第２の層の先端がカップとして形成され、その内面は、最初に所定位置に配置される際に先端空間を形成すべく取付部の先端から離間され、これにより、取り付け後における第１の層と取付部との間のその後の移動が許容されるようになっている、請求項１〜請求項７のいずれか一項に記載の大腿プロテーゼ。
- ２つの層によって形成される壁の厚さが１ｍｍ〜３ｍｍである、請求項１〜請求項８のいずれか一項に記載の大腿プロテーゼ。
- ２つの層によって形成される壁の厚さが１．８ｍｍ〜２．５ｍｍである、請求項９に記載の大腿プロテーゼ。
- 金属の第２の層の内面は、合成樹脂の第１の層の外面に対する接着を高めるために、粗面化されている、請求項１〜請求項１０のいずれか一項に記載の大腿プロテーゼ。
- ステム部を液体材料槽中に浸漬することにより或いは取付部の外面に対して材料を吹き付けることにより合成樹脂材料からなる第１の層を取り付け、その後、合成樹脂材料層上に金属をバタリングすることにより或いは選択レーザ焼結プロセスを使用することにより金属の第２の層を付加することを含む、請求項１に記載の大腿プロテーゼを形成する方法。
- 別個に形成され且つ先端−基端方向で取付部上にスライドされるシースとして第１および第２の層を設けることを含む、請求項１に記載の大腿プロテーゼを形成する方法。
