本出願は、2000年10月27日に出願された米国仮出願番号09/699,254の一部継続出願であり、順に、2000年7月11日に出願された米国仮出願番号09/613,686の一部継続出願であり、順に、2000年2月18日に出願された米国特許仮出願番号09/507,361の一部継続出願である。各出願の全体が、本明細書によって参考として援用される。
図3A〜3Jは、位置決め要素200の様々な実施形態を示す。要素200が、展開管300の内腔の範囲内に配置される場合、想定されるような、まっすぐかつ平面の構成を有する要素200の特に有用なバリエーションの透視図を図3Aに示す。
ここで図4A〜4Eについて考えると、卵型展開または送達(送出)チューブ300は、配向要素またはクロックホイール500と共に示される。
ここで、図5A〜5Cについて考えると、切断要素またはブレード600が、駆動管400の内腔において特にスライド可能に配列されて示される。前に議論したように、ブレード600は、駆動管400を介する展開にために設計されて、始めに組織を貫通し、そして送達管および1つ以上の位置決め要素200が結局展開され得るアクセス経路を作製する。
ここで、図7A〜7Cおよび8に戻って、本発明の2つの組織位置決め要素推進器アセンブリバリエーションが示される。これらのバリエーション(これらは2つであるが、多数の可能なもののうちの2つである)は、目的組織への単純で安全な反復性かつ信頼性のある位置決め要素配備を提供する目的を達成する。
図9〜20は、好ましくは、組織容積の内部の貫入または他の侵害なく、胸部からの最終的な除去または切除のための組織の容積を標識するためのシステム100を使用するための方法を、詳細に示す。1つ以上の位置決め要素が「極性」様式で展開する特に有用な技術は、以下に記載される。
一旦、所望の数の位置決め要素が、組織容積22を規定するために組織中に展開されると、この組織は、パドル30から展開され、かつ除去され、そしてシステム100の残りの成分はこの部分から除去され、その結果、この位置決め要素および任意の近位に装着された要素(例えば、可撓性ワイヤ又は縫合290)は、組織10内に残される。このことは、上記の2個の要素の展開について図202に示されている。各位置決め要素の近位部分210(または、代替的に可撓性ワイヤまたは縫合290)は、皮膚の表面を介して伸延することに注目のこと。この全位置決め要素が内部組織10であり、かつ縫合が位置決め要素の近位末端において装着される場合、この縫合は、組織10およびこの皮膚表面を介して伸延すべきであり、その結果、これにより、位置決め要素は、操作され得る。
上記の極性配置以外のアクセス点から組織10に1つ以上の位置決め要素を展開することが望ましい例があり得る。
図23Aは、エネルギーの供給源265が伝達ケーブル270、ハンドル272、およびクランプ276を介して位置決め要素に接続される代替の方法を示す。上記のように、位置決め要素にRFエネルギーで電圧を加えて、一旦、それが位置決め要素によって規定されると、身体から組織容積22を除去するための代替の手段として組織を通って切断することが好ましい例であり得る。例えば、位置決め要素200の厚みに沿った1つの縁部は、伝導性であり、露出され得(すなわち、絶縁されていない)、その結果、RFエネルギーの供給源によって電圧がかけられる場合、位置決め要素は、角度のずれを介して単一のユニットまたは「カゴ」として回転して、組織要素22を規定する組織境界を通って切断し、これを組織10の残りから除去する。角度のずれの特定の角度は、それが、もちろん存在する位置決め要素の数およびそれらの相対的な角度のずれに依存するように、組織容積10の境界を通って切断するのに必要とされる。
上で議論されたように、疑われる組織の全ての領域が単一の組織容積に含まれ得るわけではない状況が生じ得る。
本発明の以下の特徴は、予め形成された展開配置を有する位置決め要素を、これらが組織に入る際に所望の方向に展開する間に案内するかまたは進路を変更することを可能にする。これらはまた、展開の間に、このような予め形成された形状を有さない位置決め要素の、インサイチュでの冷間成型を行うことを可能にする。
患者の皮膚に位置決め要素または縫合糸290の近位部分210を付着するための特に有用な方法は、図34に示される包囲物またはポーチ110を介することである。
図35に示されるオフセット固定器具900を、1つまたは2つの位置決め要素の接線方向の展開を容易にするために設計する。本発明の他の要素(例えば、ドライバチューブ(駆動管)400、クロックホイール500など)は、明快さのために取り除いた。
これまで記載されている位置決め要素は、組織の容積の回りの経路または境界を規定することが主に意図されるが、これはまた、この容積(これはまた病変を含み得る)を貫通するように、位置決め要素を展開するための本発明の範囲内にある。この様式では、本発明は、改良した位置決め要素または位置決めワイヤと考えられ得る。
Claims (14)
- 組織位置決めデバイスであって、
第1の細長い要素(200)と第2の細長い要素(200’)とを備え、
前記第1の細長い要素(200)と前記第2の細長い要素(200’)は、展開する際に標的組織容積(22)の周囲を取り囲む曲線状の構成を成すように配置され構成される遠位固着部(220)をそれぞれ有し、
第1の細長い要素(200)の遠位固着部(220)は第1の面を規定し、第2の細長い要素(200’)の遠位固着部(220)は第2の面を規定し、
第1の面が第2の面に対して30゜から90゜の角度を付けて配置され、
標的組織容積(22)が第1の細長い要素(200)の遠位固着部(220)と第2の細長い要素(200’)の遠位固着部(220)とによって境界を定められ、
第1の細長い要素(200)の遠位固着部(220)と第2の細長い要素(220’)の遠位固着部(220)とが、それぞれ長方形断面を有するリボン形状を成し、該リボン形状は超音波によって見えることを特徴とする組織位置決めデバイス。 - 長方形断面のリボン形状は平坦面を規定し、外科医が前記第1の細長い要素(200)と前記第2の細長い要素(200’)とによって囲まれた標的組織容積(22)を切除するとき、前記平坦面が切断面となることを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記リボン形状が、標的組織容積(22)の周りで前記第1の細長い要素(200)の移動を容易にすることを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記第1の細長い要素(200)と前記第2の細長い要素(200’)は、境界を定める一方、標的組織容積(22)を貫通しないことを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記第1の面が前記第2の面に対して平行でないことを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記第2の細長い要素(200’)が、前記第1の細長い要素(200)に対して90°、45°又は30°の角度を成して配置されていることを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記第1の面及び前記第2の面に対して平行でない第3の面を有する第3の細長い要素を更に備えることを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記第3の細長い要素が、前記第1の細長い要素(200)又は前記第2の細長い要素(200’)に対して90°、45°又は30°の角度を成して配置されていることを特徴とする請求項7に記載の組織位置決めデバイス。
- 前記第1の細長い要素(200)の遠位固着部(220)が、展開した際に標的組織容積(22)の周りにループを形成することを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記第1の細長い要素(200)の遠位固着部(220)が、厚みより大きな幅を有することを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記第1の細長い要素(200)の遠位固着部(220)が、展開した後に位置決め要素の移動を最小にするために一つ以上のタブを有することを特徴とする請求項1に記載の組織位置決めデバイス。
- 前記第1の細長い要素(200)及び前記第2の細長い要素(200’)が、ステンレス鋼、ばね鋼、ニッケル−チタン合金、生体適合性ポリマーのグループから選択される材料であることを特徴とする請求項1に記載の組織位置決めデバイス。
