第三の気道の軸方向領域5410C、及びこれらの第一の気道の軸方向領域5410Aと第二の気道の軸方向領域5410Dとの間の第四の気道の軸方向領域5410Dから成っている。第一の気道の軸方向領域5410Aは、第一の局所的気道軸線5411Aを規定する。第二の気道の軸方向領域5410Bは、第二の局所的気道軸線5411Bを規定する。第三の気道の軸方向領域5410Cは、第三の局所的気道軸線5411Cを規定する。第四の気道の軸方向領域5410Dは、第四の局所的気道軸線5411Dを規定する。第一の気道の軸方向領域5410A、第二の気道の軸方向領域5410B、第三の気道の軸方向領域5410C及び第四の気道の軸方向領域5410Dは、各々、内側管腔面5413を有している。第一の気道の軸方向領域5410A、第二の気道の軸方向領域5410B、第三の気道の軸方向領域5410C及び第四の気道の軸方向領域5410Dは、互いに軸方向に連結されている。インプラント5400の第一のインプラント部分5400Aは、第一の気道の軸方向領域5410Aと係合することができる。インプラント5400の第二のインプラント部分5400Bは、第二の気道の軸方向領域5410Bと係合することができる。インプラント5400の第三のインプラント部分5400Cは、第三の気道の軸方向領域5410Cと係合することができる。インプラント5400の第四のインプラント部分5400Dは、第四の気道の軸方向領域5410Dと係合することができる。インプラント5400は、それぞれのインプラント部分を内側管腔面5413に対し押し付け、これにより気道系内にて曲りを生じさせ、且つ第一の気道の軸方向領域5410A、第二の気道の軸方向領域5410B、第三の気道の軸方向領域5410C及び(又は)第四の気道の軸方向領域5410Dの間に配設された肺組織の容量を圧縮することにより、第一の気道の軸方向領域5410A、第二の気道の軸方向領域5410B、第三の気道の軸方向領域5410C及び(又は)第四の気道の軸方向領域5410Dを互いに側方向に向けて押し付けることができる。圧縮された肺組織の容量は、十分大きく且つ十分に圧縮されて、非圧縮の肺の容量内の張力を増加させ、このため肺の肺機能が改善されるようにすることができる。
Claims (19)
- 複数の分岐する気道(5002)を有する気道系を含む患者の肺を治療するシステムにおいて、
基端と、末端と、その端部の間の管腔とを有するカテーテル(5201)であって、その末端が気道系を通して前進可能である前記カテーテル(5201)と、
前記管腔により受け入れ可能なインプラント(5300)であって、該インプラントの末端に隣接する末端部分(5303)と、インプラントの基端に隣接する基端部分(5305)とを有する細長い本体(5301)を備え、末端部分(5303)は偏倚され、カテーテル(5201)がインプラントの末端部分(5303)から基端方向に後退したとき、末端部分(5303)は解放されて気道(5002)と係合し、前記インプラントの基端は、カテーテル(5201)がインプラントの基端から基端方向に引き出される間、気道(5002)に対して末端方向に前進可能であるようにした前記インプラント(5300)と、
細長本体を有し、且つ該細長本体の長さに沿った長さ標識(5206)を有する気道測定装置(5203)であって、前記カテーテル(5201)の末端から前記気道測定装置(5203)の細長本体の末端までの末端長さが、気道(5002)内へ送り込まれるインプラント(5300)の長さを選ぶのに適した長さである、患者の肺を治療するシステム。 - 請求項1に記載のシステムにおいて、
カテーテル(5201)は気管支鏡(4902)を備える、システム。 - 請求項1に記載のシステムにおいて、
インプラント(5300)は20mm以上の長さを有する、システム。 - 請求項3に記載のシステムにおいて、
インプラント(5300)は50mmから250mmの範囲の長さを有する、システム。 - 請求項1に記載のシステムにおいて、
前記インプラント(5300、5400)は、インプラントの細長い本体(5301)の末端(5303、5400B)及び基端(5305、5400A)に隣接して配設された非外傷性表面(5402)を更に備え、該非外傷性表面(5402)は、これを取り囲む気道系と係合し、インプラント(5300、5400)の細長い本体(5301)が肺の組織を気道(5002)の内部から端部間にて圧縮するとき、該気道系を通って侵入するのを阻止し得、
該インプラント(5300、5400)は、非外傷性表面(5402)の間にて20mm以上の長さを有する、患者の肺を治療するシステム。 - 請求項5に記載のシステムにおいて、
インプラント(5300)は50mmから250mmの範囲の長さを有する、システム。 - 請求項1に記載のシステムにおいて、
患者の肺の特徴を評価する手段(5220´)を更に備える、患者の肺を治療するシステム。 - 請求項7に記載のシステムにおいて、
前記評価する手段(5220´)は、肺に連結可能であって、肺の圧縮に応答して肺の特徴を出力する肺の評価システムを具備する、患者の肺を治療するシステム。 - 請求項8に記載のシステムにおいて、
前記肺の評価手段(5220´)は、患者が肺の治療のため配置されている間、肺に連結可能であり、肺の圧縮は、同一の手順にて完了され且つ評価されることができる、ようにした、システム。 - 請求項8に記載のシステムにおいて、
前記肺の評価手段(5220´)からの肺の特徴は、肺の圧縮を完了後、数回の呼吸サイクル中、肺の圧縮に応答して、肺内の機械的な変化により実現される肺機能の改善程度を示すようにした、システム。 - 請求項8に記載のシステムにおいて、
前記肺の評価手段(5220´)は遠隔式映像化システムを備え、該遠隔式映像化システムは、肺組織の密度の少なくとも1つの変化を示し、また、肺の気道(5002)がその位置で隣接する肺組織内の張力の欠如のため、萎み易いままであることを示す、システム。 - 請求項1に記載のシステムにおいて、
前記気道測定装置(5203)の測定装置の細長本体は、
基端と、末端とを有し、
該末端は、5Fr以上の寸法を有する非外傷性表面(5402)を含む、気道の測定装置。 - 請求項1に記載のシステムにおいて、
前記インプラント(5300)の細長い本体(5301)は、気道(5002)内から隣接する肺組織を圧縮し得るよう送り込み形態から送り込まれた形態まで配備可能であり、
前記気道測定装置(5203)の細長本体は、カテーテル(5201)に沿って末端方向に伸びることができ、
前記標識(5206)は、前記インプラント(5300)の細長い本体(5301)の長さを選ぶのに適した、患者の肺を治療するシステム。 - 請求項13に記載のシステムにおいて、
前記カテーテル(5201)は側方向に可撓性であり、該カテーテル(5201)の管腔は、インプラント(5300)を送り込む前、該インプラントの細長い本体(5301)を送り込み形態に維持し得るよう、挿入中、該インプラントの細長い本体(5301)を解放可能に受け入れ、
送り込み形態にある前記インプラントの細長い本体(5301)は、該細長い本体(5301)の基端と該細長い本体(5301)の末端との間にて軸線に沿って伸びることが可能であり、
前記インプラント(5300)を送り込み形態から送り込まれた形態まで配備するステップは、端部の間にてインプラント(5300)を曲げて、該インプラント(5300)を曲げることにより、気道(5002)が軸線から離れるように曲げられた形態になるようにするステップを含む、システム。 - 請求項13に記載のシステムにおいて、
前記インプラントの細長い本体(5301)は、偏倚されて、曲がった配備した形態になるように曲げられ、前記管腔は、前記インプラントの細長い本体(5301)をカテーテル(5201)内にて規制することにより送り込み形態に維持する、システム。 - 請求項15に記載のシステムにおいて、
複数の代替的に選択可能なインプラント(5300)を更に備え、
該インプラント(5300)の各々は、細長い本体(5301)を備え、且つ格納中、細長い本体(5301)の歪みを制限し得るよう曲がった形態に向けて解放され、
インプラント(5300)の各々は、関係した細長い本体(5301)を軸方向に真直にすることにより、且つカテーテル(5201)が細長い本体(5301)を送り込み形態に維持するよう細長い本体(5301)を管腔内に挿入することにより、カテーテル(5201)内に装填可能である、システム。 - 請求項16に記載のシステムにおいて、
基端と末端とを有する管状の装填カートリッジ(5401)を更に備え、
該装填カートリッジ(5401)は、複数の細長い本体(5301)から選んだ細長い本体(5301)を解放可能に受け入れ、
前記装填カートリッジ(5401)は、選んだ細長い本体(5301)がカテーテル(5201)の管腔内に装填カートリッジ(5401)内から末端方向に前進させることができるよう前記カテーテル本体(5201)と連結可能であり、前記装填カートリッジ(5401)の末端は、該装填カートリッジ(5401)の管腔がカテーテル(5201)の管腔まで滑らかに伸びるよう前記カテーテル(5201)の基端側ハブ(3802)に固定可能であり、
前記細長い本体(5301)を、装着した装填カートリッジ(5401)からカテーテル(5201)内まで押すことができるよう前記装填カートリッジ(5401)及びカテーテル(5201)内にて軸方向に可動のプッシャ(5009)を更に備え、
該プッシャ(5009)は、インプラント(5300)に対して末端方向に係合可能なプッシャ面(5010)と、該プッシャ面(5010)から基端方向に伸びてインプラント(5300)をカテーテル(5201)の末端から送り込み易くする軸(5009)とを有する、システム。 - 請求項13に記載のシステムにおいて、
基端と、末端と、両端部の間の管腔とを有する気管支鏡(4902)と、像信号送信機(4903、5004、5006、5007)とを更に備え、
カテーテル(5201)の末端は、管腔内に受け入れ可能であり、また、気管支鏡(4902)の視界を越えて末端方向に前進可能であり、インプラント(5300)を気管支鏡(4902)の視界を越えて末端方向に送り込むことができ、
前記気道測定装置(5203)の細長本体は、気管支鏡(4902)の管腔を通って気道(5002)の末端まで末端方向に前進可能なガイドワイヤー(5203)であって、気管支鏡(4902)とガイドワイヤーの末端(5209)との間のガイドワイヤーの末端方向長さの標識(5206)を有する前記ガイドワイヤー(5203)を備える、システム。 - 請求項13に記載のシステムにおいて、
前記インプラント(5300)は、複数の代替的に選択可能なインプラント(5300A〜5300C)であって、インプラントの各々は、細長い本体(5301)を備え、且つ格納中、細長い本体(5301)の歪みを制限し得るよう曲がった形態に向けて解放され、
該インプラント(5300A〜5300C)の各々は、関係した細長い本体(5301)を軸方向に真直にすることにより、且つカテーテル(5201)が細長い本体(5301)を送り込み形態に維持するよう細長い本体(5301)を管腔内に挿入することにより、カテーテル(5201)内に装填可能である、システム。
