Description
患者の脈管構造に経皮的にアクセスするための、例えば脈管内で作業を行うため、および作業完了後に生じる穿刺を閉塞するための器具および方法が知られている。例えば、中空針を患者の皮膚およびその被覆組織を通して血管内に挿入することができる。ガイドワイヤを針管腔から血管内に通すことができ、その後針を取り除くこともできる。次に誘導シースをガイドワイヤ上に沿って進め、例えば一緒に、もしくは続けて一つまたは複数の拡張器を血管内に通すこともできる。医療処置を行うために、カテーテルまたはその他装置を誘導シースの中およびガイドワイヤの上をある位置まで進めることもできる。このように誘導シースは、脈管装置の血管内へのアクセスおよび/または導入を容易にすると同時に、血管壁の外傷を最小限にし、かつ/または失血を最小限にする。処置終了後に装置および誘導シースは除去され、皮膚と血管壁の間に伸びる穿刺が残ることがある。
発明は、体内の穿刺を閉塞するための器具、より具体的には血管に伸びる脈管穿刺内に一時的もしくは永久的な止血を提供するための器具、および/または患者の皮膚から血管もしくはその他体部管腔に伸びる経皮的穿刺の中に閉塞栓を送達するための器具を目的とする。
図面に転ずると、図1A〜1Dは組織(図示せず)を貫通して伸びる穿刺を閉塞するための装置2を描いている。一般的には、装置2はその上に第一ヒドロゲル前駆体6および第二ヒドロゲル前駆体7を配した、例えば栓の形をした担体またはコア4を具備する。第一および第二ヒドロゲル前駆体6、7は、担体4の上に非反応状態で配される。第一および第二ヒドロゲル前駆体6、7は、例えば水性の生理的環境に曝される前もしくは曝されるまでは非反応状態を保つことができる。水性の生理的環境は、例えば組織を貫通して伸びる穿刺跡の内側に存在するだろう。
Claims (17)
- 組織を貫通して伸びる穿刺を閉塞するための器具であって:
基端部、組織を貫通している穿刺内への挿入に適した大きさの先端部、および前記基端部と先端部の間に伸びる管腔を具えるカートリッジと;
前記カートリッジの管腔内に配置された栓であって、その基端部と先端部の間を伸びる管腔を具備している栓と;
前記カートリッジから前記栓を展開するために前記カートリッジをプッシャ部材に対して近位に引き込んだときに、前記栓が近位へ移動することを防止するために前記カートリッジ管腔内に配置された、プッシャ部材と;
細長の部材と、前記細長の部材の先端部に担持された拡張可能な位置決め要素と、折り畳まれた状態と拡張状態の間で前記位置決め要素を拡張させるための前記細長の部材の基端部におけるアクチュエータとを有する位置決め部材であって、前記栓が前記位置決め要素のすぐ近くに配置されるように、前記細長の部材を前記栓とカートリッジの管腔を通して受けている、位置決め部材と;
前記プッシャ部材が遠位位置に前進したときに前記位置決め要素に対し前記プッシャ部材が近位に引き込まれることを防止するために前記プッシャ部材の戻り止めとして機能する、前記プッシャ部材と前記位置決め部材上の協働エレメントと;を含み、
前記位置決め部材が折り畳まれた状態の位置決め要素と共にプッシャ部材を介して取り外し可能であり、それによって、プッシャ部材を取り除く前に位置決め部材を穿刺から取り外すことができ、その結果、位置決め部材が取り除かれる間に前記プッシャ部材が前記栓が近位へ移動することを防ぐことを特徴とする器具。 - 組織を貫通して伸びる穿刺を閉塞するための器具であって:
基端部、組織を貫通している穿刺内への挿入に適した大きさの先端部、および前記基端部と先端部の間に伸びる管腔を具えるカートリッジと;
前記カートリッジの管腔内に配置された栓であって、その基端部と先端部の間を伸びる管腔を具備している栓と;
前記カートリッジから前記栓を展開するために前記カートリッジをプッシャ部材に対して近位に引き込んだときに、前記栓が近位へ移動することを防止するために前記カートリッジ管腔内に配置されたプッシャ部材と;
細長の部材と、前記細長の部材の先端部に担持された拡張可能な位置決め要素と、折り畳まれた状態と拡張状態の間で前記位置決め要素を拡張させるための前記細長の部材の基端部におけるアクチュエータとを有する位置決め部材であって、前記カートリッジ先端部および栓が前記位置決め要素のすぐ近くに配置され、それにより前記カートリッジ先端部および栓が位置決め部材と共に穿刺内に導入可能となるように、前記細長の部材が前記栓とカートリッジの管腔を通って収容されている、位置決め部材と;
前記プッシャ部材が遠位位置に前進したときに前記位置決め要素に対し前記プッシャ部材が近位に引き込まれることを防止するために前記プッシャ部材の戻り止めとして機能し、折り畳まれた状態の位置決め要素と共にプッシャ部材を介して位置決め部材を除去可能とする、前記プッシャ部材と前記位置決め部材上の協働エレメントと;を含むことを特徴とする器具。 - 組織を貫通して伸びる穿刺を閉塞するための器具であって:
基端部、誘導シース内への挿入に適した大きさの先端部、および前記基端部と先端部の間に伸びる管腔を具える管状部材と;
前記管状部材の管腔内に配置された栓であって、その基端部と先端部の間を伸びる管腔を具備している栓と;
前記栓を前記管状部材から展開するために前記管状部材をプッシャ部材に対して近位に引き込んだときに、前記栓が近位へ移動することを防止するために前記管状部材の管腔内に配置された、基端部および先端部を具えるプッシャ部材であって、当該プッシャ部材の管腔はプッシャ部材の基端部と先端部の間に伸びているプッシャ部材と;
細長の部材と、前記細長の部材の先端部に担持された拡張可能な位置決め要素と、折り畳まれた状態と拡張状態の間で前記位置決め要素を拡張させるための前記細長の部材の基端部におけるアクチュエータとを有する位置決め部材であって、前記栓およびプッシャ部材の先端部が前記位置決め要素のすぐ近くに配置され、それにより前記栓およびプッシャ部材の先端部が前記位置決め部材と共に穿刺内に導入可能となるように、前記細長の部材が前記栓とプッシャ部材の管腔を通って収容されている、位置決め部材と;
前記プッシャ部材が遠位位置に前進したときに前記位置決め要素に対し前記プッシャ部材が近位に引き込まれることを防止し、前記位置決め部材を折り畳まれた状態の位置決め要素と共にプッシャ部材に対して近位に引き込み、前記位置決め要素を前記栓を通って前記プッシャ部材内に引き込むことができるようにする、前記プッシャ部材の戻り止めとして機能する協働エレメントと;を含むことを特徴とする器具。 - 戻り止めとして機能する前記協働エレメントが、前記プッシャ部材の上のラッチ要素と、前記位置決め部材の上の隆起要素とを具える、請求項1乃至3の何れか1項に記載の器具。
- 前記アクチュエータが、前記細長い部材を通って前記位置決め要素まで延在している管腔に結合している膨張供給源を具える、請求項1乃至3の何れか1項に記載の器具。
- 前記アクチュエータがプルワイヤを具える、請求項1乃至3の何れか1項に記載の器具。
- 戻り止めとして機能する前記協働エレメントが前記プッシャ部材の基部方向への移動を防ぐように作用し、前記位置決め要素が拡張状態にあるときに前記プッシャ部材と前記位置決め要素の間で前記栓が圧縮されるように、前記プッシャ部材を前記位置決め要素に対して遠位側へ方向付ける、請求項1乃至3の何れか1項に記載の器具。
- 前記栓が、加水分解により分解可能な結合を含む凍結乾燥ヒドロゲルで構成され、前記ヒドロゲルは水性の生理的環境に曝されると拡張する、請求項1乃至7のいずれか1項に記載の器具。
- 前記栓が、折りたたみシートの円筒形を具える、請求項1乃至8のいずれか1項に記載の器具。
- 前記管状部材がカートリッジである、請求項3に記載の器具。
- 前記位置決め部材が折り畳まれた状態の前記位置決め要素と共に前記プッシャ部材を通って除去可能であり、戻り止めとして機能する前記協働エレメントは前記プッシャ部材の基部方向への移動を防ぐように作用し、これによって、前記プッシャ部材を引き込む前に穿刺から前記位置決め部材を引き込むことができ、前記位置決め部材を引き込む間に前記プッシャ部材は前記栓が近位側へ移動することを防止する、請求項1、2および10のいずれか1項に記載の器具。
- 前記カートリッジが前記プッシャ部材に対して引き込み可能であり、前記プッシャ部材が、前記カートリッジを引き込む際に前記栓が近位側に移動することを防止する遠位端を具える、請求項11に記載の器具。
- さらに、基端部と、組織を貫通する穿刺内への挿入に適した大きさの先端部と、内側にカートリッジを受けるために前記基端部と先端部の間に伸びる管腔とを具える誘導シースを具える、請求項1、2、および10乃至12のいずれか1項に記載の器具。
- 前記栓が、円盤形、中実の円筒形、圧延シートの円筒形、または折りたたみシートの円筒形をした細長い本体を具える、請求項1乃至8のいずれか1項に記載の器具。
- 前記位置決め部材が、近位位置において、前記位置決め要素が穿刺内へ前進したとき前記カートリッジが穿刺の外側に配置される長さを有している、請求項1、2、および10乃至13のいずれか1項に記載の器具。
- 組織を貫通して伸びる穿刺を閉塞するための器具であって:
基端部、組織を貫通している穿刺内への挿入に適した大きさの先端部、および前記基端部と先端部の間に伸びる管腔を具えるカートリッジと;
前記カートリッジの管腔内に配置された栓であって、その基端部と先端部の間を伸びる管腔を具備している栓と;
前記カートリッジをプッシャ部材に対し引き込んだとき、前記カートリッジから栓を展開するために前記カートリッジ管腔内に配置されたプッシャ部材と;
細長の部材と、前記細長の部材の先端部に担持された拡張可能な位置決め要素と、折り畳まれた状態と拡張状態の間で前記位置決め要素を拡張させるための前記細長の部材の基端部におけるアクチュエータとを有する位置決め部材であって、前記カートリッジ、栓、及びプッシャ部材が前記位置決め部材の近位位置から遠位位置へ前進することができ、前記栓が前記位置決め要素の近傍に配置されるように、前記細長の部材を、前記栓とカートリッジの管腔を通して受けている、位置決め部材と;
前記プッシャ部材が前記遠位位置に前進したときに前記位置決め部材に対し前記プッシャ部材が基部方向に動くことを防止するために前記プッシャ部材の戻り止めとして機能する、前記プッシャ部材と前記位置決め部材上の協働エレメントと;を含み、
前記位置決め部材が折り畳まれた状態の位置決め要素と共にプッシャ部材を介して取り外し可能であり、それによって、プッシャ部材を取り除く前に位置決め部材を穿刺から取り外すことができ、その結果、前記位置決め部材が取り除かれる間に前記プッシャ部材は前記栓が近位へ移動することを防ぐことを特徴とする器具。 - 組織を貫通して伸びる穿刺を閉塞するための器具であって:
細長の部材を具える位置決め部材であって、前記細長の部材は、その先端部の拡張可能な位置決め要素と、折り畳まれた状態と拡張状態の間で前記位置決め要素を拡張させるためのアクチュエータを有する基端部のハウジングとを有している位置決め部材と;
基端部、組織を貫通している穿刺内への挿入に適した大きさの先端部、および前記基端部と先端部の間に伸びる管腔を具えるカートリッジと;
前記カートリッジの管腔内に配置された栓であって、その基端部と先端部の間を伸びる管腔を具備している栓と;
前記カートリッジがプッシャ部材に対して引き込まれたときに、前記栓を前記カートリッジから展開するために前記カートリッジ管腔内に配置されたプッシャ部材と;を含み、
前記カートリッジは、前記細長の部材を前記栓およびカートリッジの管腔を通って受けた状態で前記細長の部材の基端部に配置され、これにより、前記カートリッジ、栓、及びプッシャ部材は、前記位置決め要素から近位方向に離れているとともに、前記位置決め部材の基端部から遠位位置へ前進可能であり、前記カートリッジの先端部および栓は前記位置決め要素の近傍に配置され、
前記プッシャ部材が前記遠位位置に前進したときに、前記位置決め部材に対し前記プッシャ部材が近位方向に動くことを防止する、前記プッシャ部材と前記位置決め部材上の前記プッシャ部材の戻り止めとして機能する協働エレメントと;を含むことを特徴とする器具。
