Description

外科用装置１０のコンポーネントは概して、ポリカーボネートを含む熱可塑性物質と、ステンレス鋼およびアルミニウムを含む金属とから形成される。特定のコンポーネントを形成するために選択される特定の材料は、その特定のコンポーネントの強度要件に依存する。ステープル７は従来のタイプであり、バックスパンを含み、このバックスパンは、バックスパンから延びる一対のレグを有する。レグは、組織貫通先端部で終わる。

図８〜図１０を参照すると、外科用ステープリング装置１０が、例えば２つの相対する腸管区域の接合をもたらすための吻合処置で使用される様子が示されている。吻合処置は通常、腹腔鏡的手段および器具使用を含む低侵襲性の手術技術を使用して実行される。最初、罹患した腸管区域が除去される。その後、アンビルアセンブリ６０が手術用切開または肛門のいずれかを通して手術部位に挿入され、腸管区域「Ｔ１」内に位置決めされる。ステープルカートリッジアセンブリ７０を含む、外科用ステープリング装置１０の細長い本体部分３０が、肛門を通して他方の腸管区域「Ｔ２」の中に挿入される。腸管区域「Ｔ１」および「Ｔ２」は次に、例えば巾着縫合「Ｐ」（図９を参照）のような従来の手段により、それらのそれぞれのコンポーネント（例えば、アンビルアセンブリ６０のシャフト６４および細長い本体部分３０の遠位端）の周りに一時的に固定される。
その後、臨床医は、シャフト６４の近位端が、外科用ステープリング装置１０の細長い本体部分３０に配置された接近シャフト７５の遠位端の中に挿入され、そして、これに取り付けられる／接続されるまで、アンビルアセンブリ６０を操作する。シャフト６４は、単一の構造として動作可能であるように接近シャフト７５を係合する。アンビルアセンブリ６０および細長い本体部分３０は次に、腸管区域「Ｔ１」および「Ｔ２」を近づけるために接近させられる。外科用ステープリング装置１０が次に発射され、各々がそれぞれのステープル受け入れスロット７２に配置された複数のステープルプッシャ９にそれぞれのステープル７をスロット７２を通して排出させる。ステープル７は、腸管区域「Ｔ１」および「Ｔ２」ならびにバットレス部材１１２を貫いてアンビルアセンブリ６０の方に移動し、それによって、腸管区域「Ｔ１」および「Ｔ２」が互いとステープル留めし合うようにする。一方、円筒形のナイフ７６は、円筒形のナイフ７６の半径方向内側に配置された組織の一部分をカットし、吻合を完了する。このとき、タブ１２０は、腸管区域「Ｔ１」および「Ｔ２」にステープル留めされたバットレス部材１１２に取り付けられたままであり、環状リング１３０によってステープルカートリッジアセンブリ７０に固定されている。外科用ステープリング装置１０が手術部位から除去されるとき、バットレス部材１１２に隣接する破断または脆弱線が、腸管区域「Ｔ１」および「Ｔ２」にステープル留めされたバットレス部材１１２からのタブ１２０の分離を容易にする。
ここで、図１１〜図１３を参照すると、本開示の別の実施形態によるバットレスアセンブリ３００が示されている。締め付け部材は、縫合糸３３０の形態である。特に、バットレスコンポーネント３１０は、ステープルカートリッジアセンブリ３７０の遠位表面３７４上に位置決めされるような構成および寸法とされたバットレス部材３１２と、バットレス部材３１２の外側の放射状エッジから近位方向に延びるリム３２０とを含む。リム３２０は、ステープルカートリッジアセンブリ３７０の外壁３７９を係合し、それによって、バットレスコンポーネント３１０は、ステープルカートリッジアセンブリ３７０の遠位表面３７４を包む。円筒形のナイフ３７６は、バットレス部材３１２の半径方向内側に配置され、そのような構成のもとで、円筒形のナイフ３７６によるバットレス部材３１２の切断はなくなる。

図１４〜図１６を参照すると、本開示の別の実施形態によるバットレスアセンブリ４００が示されている。バットレスアセンブリ４００は、バットレスコンポーネント４１０と、縫合糸４３０の形態での締め付け部材とを含む。特に、バットレスコンポーネント４１０は、ステープルカートリッジアセンブリ４７０の遠位表面４７４上に位置決めされるような構成および寸法とされたバットレス部材４１２と、バットレス部材４１２の外側の放射状エッジから近位方向に延びるリム４２０とを含む。リム４２０は、ステープルカートリッジアセンブリ４７０の外壁４７９を係合し、それによって、バットレスコンポーネント４１０は、ステープルカートリッジアセンブリ４７０の遠位表面４７４を包む。

各ボア４１５は、接着剤または、例えばヒドロゲルのようなゲルを含み得、ボア４１５における縫合糸４３０の固定を向上させることがさらに勘案されている。さらに、縫合糸４３０は、複数の、例えば一方向性のかかり（ｂａｒｂ）を含み得、ボア４１５における縫合糸４３０の固定を向上させることも想定されている。かかり付き縫合糸の例は、その全内容が参考として本明細書に援用されている２００９年１月２９日に出願された米国特許出願第１２／３６１，９６２号に開示されている。複合のかかり付き縫合糸は、Ｔｙｃｏ Ｈｅａｌｔｈｃａｒｅ Ｇｒｏｕｐ，ＬＬＰ（ｄｂａ Ｃｏｖｉｄｉｅｎ ＡＧ、マンスフィールド、マサチューセッツ）からＶ−ＬＯＣＴＭとして商業的に入手可能である。
図１７〜図１９を参照すると、本開示の別の実施形態によるバットレスアセンブリ６００が示されている。バットレスアセンブリ６００は、バットレス部材６１０と、バットレス部材６１０の外側の放射状エッジから近位方向に延びる複数の円周に沿って配列されたタブ６２０とを含む。各タブ６２０は、内壁またはその表面６２５に取り付け部材６３０ａを含む。
さらに、ステープルカートリッジアセンブリ６７０の外壁６７９は、タブ６２０上の取り付け部材６３０ａと対応する複数の円周に沿って配列された取り付け部材６３０ｂを含む。取り付け部材６３０ａおよび６３０ｂは、例えば、再締め付け可能なテープまたはフックおよびループ締め付け具を含み得る。あるいは、タブ６２０は、バットレスアセンブリ６００をステープルカートリッジアセンブリ６７０に分離可能に位置決めするための両面接着テープを含み得る。
さらに、各タブ６２０は、破断、ミシン目、または脆弱線６１６を含み得、それによって、組織にステープル留めされるバットレス部材６１０が、タブ６２０から切断または分離され得ることも想定されている。このようにして、タブ６２０は、取り付け部材６３０ａおよび６３０ｂによってステープルカートリッジアセンブリ６７０に取り付けられ得、そして、ステープルカートリッジアセンブリ６７０と共に手術部位から除去され得る。
ここで、図２０〜図２２を参照すると、バットレスアセンブリ７００は、バットレス部材７１０と、ステープルカートリッジアセンブリ７７０の内壁７７７を係合する、バットレス部材７１０の内側の放射状エッジから近位方向に延びる複数の円周に沿って配列されたタブ７２０とを含み得ることも勘案されている。特に、各タブ７２０は、その外壁に取り付け部材７３０ａを含む。さらに、ステープルカートリッジアセンブリ７７０の内壁７７７は、タブ７２０上の取り付け部材７３０ａと対応する複数の円周に沿って配列された取り付け部材７３０ｂを含む。取り付け部材７３０ａおよび７３０ｂは、例えば、再締め付け可能なテープまたはフックおよびループ締め付け具を含み得る。
このような構成のもとで、円筒形のナイフ７７６は、ステープルカートリッジアセンブリ７７０の内壁７７７に円周に沿って配列された取り付け部材７３０ｂの半径方向内側に位置決めされ、そして、各タブ７２０のそれぞれの外壁の取り付け部材７３０ａの半径方向外側に位置決めされる。このような構成のもとで、円筒形のナイフ７７６は、取り付け部材７３０ａと７３０ｂとの間を移動し、タブ７２０をバットレス部材７１０から切断する。さらに、タブ７２０は、タブ７２０をバットレス部材７１０から切断することを容易にするための破断、ミシン目、または脆弱線７１６を含み得る。このようにして、組織にステープル留めされるバットレス部材７１０は、タブ７２０から分離され得る。
図２３および図２４を参照すると、本開示の別の実施形態によるバットレスアセンブリ８００が示されている。本明細書に上述されたバットレスアセンブリ１００、２００、３００、４００、６００、７００とは対照的に、バットレスアセンブリ８００は、アンビルアセンブリ６０に確実に位置決めされるように構成され、かつ適合される。バットレスアセンブリ８００は、バットレスコンポーネント８１０を含む。縫合糸８３０が、バットレスコンポーネントをステープリング装置に取り付けるために使用される。バットレスコンポーネント８１０は、アンビル部材６２に取り付けられたときアンビル部材６２の外周エッジと同一平面となるような構成および寸法とされる環状プロファイルを有するバットレス部材８１２と、バットレス部材８１２から遠位方向に延びるリム８２０とを含む。さらに、リム８２０は、アンビル部材６２の外壁６１を係合し、それによって、バットレスコンポーネント８１０は、ステープル７を受け入れこれらを形成するための複数のポケットを画定するアンビル部材６２の近位表面を包む。さらに、バットレス部材８１２は、そこを通してアンビルアセンブリ６０のシャフト６４を受け入れるような構成および寸法とされるアパーチャ、ホール、またはボア８１６を画定する。ボア８１６は、シャフト６４の周りに摩擦および締りばめを提供するような大きさとされる。さらにまたは代替として、縫合糸８３０が、バットレス部材８１２をシャフト６４に確実に締め付けるためにさらに利用され得る。
図２５および図２６を参照すると、代替の実施形態において、バットレスアセンブリ９００は、バットレス部材９１２を含み、バットレス部材９１２は、外側リム９２０と内側リム９４０との間で半径方向に延びている。特に、外側リム９２０は、アンビル部材６２の外壁６１を係合するような構成および寸法とされ、内側リム９４０は、アンビルアセンブリ６０のシャフト６４に対する締りばめまたは摩擦ばめを提供するような構成および寸法とされる。このようにして、アンビルアセンブリ６０に対するバットレスアセンブリ９００の半径および長手方向の動きは、最小とされる。さらに、縫合糸９３０の形態での締め付け部材は、バットレス部材９１２の内側リム９４０の周りでアンビルアセンブリ６０のシャフト６４に対して巻かれるか、または結び付けられ、バットレスアセンブリ９００をアンビルアセンブリ６０上にさらに確実に固定する。
図２５を引き続き参照すると、バットレスアセンブリ９００は、ステープルカートリッジアセンブリ７０の遠位表面７４に位置決め可能であるバットレスアセンブリ１００、および本明細書に上述されたバットレスアセンブリ２００、３００、４００、６００、７００のうちの任意のものと共に使用され得る。バットレスアセンブリ１００は、円筒形のナイフ７６の半径方向外側に位置決めされる。従って、ハンドル部材２２が作動すると、バットレス部材１１２全体が組織にステープル留めされて組織を補強し、円筒形のナイフ７６によってバットレス部材１１２を切断することはなくなる。しかしながら、円筒形のナイフ７６は、内側リム９４０および内側リム９４０の周りに巻かれかつ結び付けられた縫合糸９３０と共に、バットレス部材９１２の一部分の芯抜きをすることにより、そのような部分を組織に同じくステープル留めされているバットレス部材９１２の一部分から解き放つ。アセンブリ２００、３００、４００、６００、７００、８００、９００の使用および動作は、本明細書に上述されたバットレスアセンブリ１００の使用および動作と実質的に同様である。従ってここでさらには記述されない。
本開示の例示的な実施形態が、添付の図面を参照して本明細書に記述されたが、上記の説明、開示、および図面は、限定するものとして解釈されるべきではなく、特定の実施形態の単なる例示として解釈されるべきである。例えば、内側リム９４０と外側リム９２０との間で半径方向に延びるバットレス部材９１２を含むバットレスアセンブリ９００は、ステープルカートリッジアセンブリ７０の遠位表面７４上での使用に対して調整され得る。さらに、バットレスアセンブリ９００は、アンビルアセンブリ６０の外壁６１に対してバットレスアセンブリ９００の外側リム９２０を固定するために、環状リング１３０または縫合糸３３０をさらに利用し得る。従って、本開示は、これらの明確な実施形態に限定されず、本開示の範囲または精神から逸脱することなく当業者によってそれらの明確な実施形態において様々な他の変更および改変がなされ得ることが理解されるべきである。

Claims (14)

  1. 外科用ステープルの環状アレイで２つの中空器官区域を接合するための装置であって、前記装置は、
    環状アレイ内に複数の外科用ステープルを含むステープルカートリッジコンポーネントと、
    前記複数の外科用ステープルを形成するための複数のステープルポケットを画定するアンビル部材と、前記アンビル部材から遠位方向に延びているシャフトとを含むアンビルコンポーネントであって、前記アンビルコンポーネントは、前記ステープルカートリッジコンポーネントと前記アンビルコンポーネントとの間で前記器官区域を調節可能に締め付けるために、離れた位置と接近した位置との間で前記ステープルカートリッジコンポーネントに対して可動である、アンビルコンポーネントと、
    前記ステープルカートリッジコンポーネントにおいて同心的に配列されたナイフ部材であって、前記ナイフ部材は、前記ステープルカートリッジコンポーネントに対して可動である、ナイフ部材と、
    前記アンビルコンポーネントと同心的に整列されたバットレス部材と、前記バットレス部材から遠位方向に延びている外側リムとを含む第１のバットレスコンポーネントであって、前記第１のバットレスコンポーネントは、前記アンビル部材内に画定された前記複数のステープルポケットを含み、前記第１のバットレスコンポーネントは、複数のボアを備える、第１のバットレスコンポーネントと、
    前記第１のバットレスコンポーネントを前記アンビルコンポーネントに固定するための複数のバーブ（ｂａｒｂ）を含む縫合糸である第１の締め付け部材であって、前記縫合糸は、前記第１のバットレスコンポーネントの前記ボアを通過し、前記複数のバーブは、前記第１のバットレスコンポーネントのボアにおいて前記縫合糸を固定する、第１の締め付け部材と
    を含む、装置。
  2. 前記外側リムは、前記アンビル部材の外側表面に係合する、請求項１に記載の装置。
  3. 前記第１のバットレスコンポーネントは、前記バットレス部材から遠位方向に延びている内側リムをさらに含み、前記内側リムは、前記アンビルコンポーネントのシャフトに係合するような構成および寸法とされている、請求項１に記載の装置。
  4. 前記ステープルカートリッジコンポーネントと同心的に整列された第２のバットレスコンポーネントをさらに含む、請求項１に記載の装置。
  5. 前記第２のバットレスコンポーネントは、バットレス部材と、前記バットレス部材から近位方向に延びている円周に沿って配列された複数のタブとを含む、請求項４に記載の装置。
  6. 前記複数のタブは、前記ステープルカートリッジコンポーネントの外壁に係合する、請求項５に記載の装置。
  7. 前記複数のタブと、前記ステープルカートリッジコンポーネントの前記外壁とは、前記第２のバットレスコンポーネントを前記ステープルカートリッジコンポーネントに固定するための対応するフックおよびループ締め付け具を含む、請求項６に記載の装置。
  8. 各タブは、前記複数のタブからの前記バットレス部材の分離のための、前記バットレス部材に隣接する脆弱線を含む、請求項６に記載の装置。
  9. 前記第２のバットレスコンポーネントは、前記複数のタブが前記ステープルカートリッジコンポーネントの内壁に係合するように構成されている、請求項５に記載の装置。
  10. 前記複数のタブと前記ステープルカートリッジコンポーネントの前記内壁とは、前記第２のバットレスコンポーネントを前記ステープルカートリッジコンポーネントに固定するための対応するフックおよびループ締め付け具を含む、請求項９に記載の装置。
  11. 各タブは、前記複数のタブからの前記バットレス部材の分離のための、前記バットレス部材に隣接する脆弱線を含む、請求項９に記載の装置。
  12. 前記ステープルカートリッジコンポーネント上に前記第２のバットレスコンポーネントを確実に位置決めするために、前記円周に沿って配列された複数のタブに係合するような構成および寸法とされた第２の締め付け部材をさらに含む、請求項５に記載の装置。
  13. 前記第２の締め付け部材は、環状リングであり、前記環状リングは、前記ステープルカートリッジコンポーネントの外壁を背にした前記円周に沿って配列された複数のタブに対して内向きの力を及ぼすような構成および寸法とされている、請求項１２に記載の装置。
  14. 前記第２の締め付け部材は、前記ステープルカートリッジコンポーネントの外壁を背にした前記円周に沿って配列された複数のタブの周りで結び付けられている縫合糸である、請求項１２に記載の装置。
