Description
Claims (17)
- 自己拡張型ステント(2)を送達するための腔内装置(1)であって、遠位側から近位側まで長手方向軸線に沿って延在し、送達構成において、
a)長手方向軸線に沿った内腔(7)を有し、前記内腔(7)の遠位端に位置するステント受入れ領域(8)を摺動可能に覆う撤回式シース(5)と、
b)前記腔内装置(1)内で長手方向及び中央に配設され、少なくとも遠位部分が前記シース(5)内に配設されている内軸(4)と、
c)前記内腔(7)の前記ステント受入れ領域(8)内に配設された圧縮状態の自己拡張型ステント(2)と、
d)内側キャビティ(9)の境界を定め、前記シース(5)内にある前記ステント(2)の近位側に隣接して配設され、近位端が前記内軸(4)に恒久的に接合されている、保持手段(3)と、
e)前記内腔(7)の近位側に向かって位置し、前記撤回式シース(5)を前記内軸(4)に対して長手方向で変位させることができる、ハンドリング手段と、を備えている腔内装置(1)において、
前記保持手段(3)が、前記腔内装置(1)内における圧縮状態から拡張状態へと径方向で拡張することができる自己拡張性を有し、
前記保持手段(3)が、弾性材料で作られた複数のフィラメントを編組又は織成することによって形成され、
前記拡張状態では、前記保持手段(3)の前記キャビティ(9)の直径がその遠位端に向かって径方向で増大し、
前記腔内装置(1)が前記送達構成のとき、前記ステント(2)の近位部分を取り囲む前記保持手段(3)の遠位部分が、前記保持手段(3)と前記ステント(2)との間の重なり領域(10)を規定し、
前記重なり領域(10)の長さL(10)と圧縮状態における前記自己拡張型ステント(2)の長さL(2)Compとの比L(10)/L(2)Compが、圧縮状態において、少なくとも10%、最大で30%であることを特徴とする腔内装置。 - 前記内軸(4)が、前記内軸(4)の一つの部分を取り囲む径方向突出部(19)を更に備え、前記径方向突出部(19)が、前記保持手段(3)の前記内側キャビティ(9)内にある前記ステント(2)の外側でその近位端に隣接して位置決めされる、請求項1に記載の腔内装置。
- 前記径方向突出部(19)の外径が、前記撤回式シース(5)の内径の少なくとも35%、最大で80%である、請求項2に記載の腔内装置。
- 拡張したときの前記保持手段(3)が、
(i)U字形、
(ii)切頭円錐状、又は、
(iii)遠位端に円筒状部分を有する切頭円錐状
のいずれか一つを取る、請求項1から3のいずれか一項に記載の腔内装置。 - 前記保持手段(3)が多層の編組から形成される、請求項1から4のいずれか一項に記載の腔内装置。
- 前記保持手段(3)が、角度(α)を間に形成する編組フィラメントで形成され、前記角度が、拡張したときの前記保持手段(3)の少なくとも遠位部分において、少なくとも120°である、請求項1から5のいずれか一項に記載の腔内装置。
- それぞれの拡張状態において、遠位端における前記保持手段(3)の直径Φ(3)Expが前記ステント(2)の直径Φ(2)Expよりも大きい、請求項1から6のいずれか一項に記載の腔内装置。
- 前記保持手段(3)の前記フィラメントの前記弾性材料が、ニチノール(登録商標)、並びにエルジロイ(登録商標)及びフィノックス(登録商標)などのコバルト系合金から成る群から選択される、形状記憶材料である、請求項1から7のいずれか一項に記載の腔内装置。
- 前記自己拡張型ステント(2)が、フィラメントの編組又は織成によって得られるメッシュ状構造を含む、請求項1から8のいずれか一項に記載の腔内装置。
- 前記メッシュ状構造が多層の編組である、請求項9に記載の腔内装置。
- 前記保持手段(3)の前記キャビティ(9)内に配設されているメンブレン(12)を更に備え、前記メンブレン(12)の近位端が、前記キャビティ(9)の底部に隣接して前記内軸(4)に恒久的に結合され、前記メンブレン(12)の遠位部分が前記ステント(2)の近位部分の周りに配設されている、請求項1から10のいずれか一項に記載の腔内装置。
- 前記撤回式シース(5)及び前記保持手段(3)が、前記シース(5)及び前記保持手段(3)の遠位端の周りに放射線不透過性材料を含む、請求項1から11のいずれか一項に記載の腔内装置。
- 前記内軸(4)が、前記突出部(19)の遠位端よりも遠位側で前記内軸(4)と前記ステント(2)との間に位置する、少なくとも一つの補足の突出部(20)を更に備えている、請求項2から12のいずれか一項に記載の腔内装置。
- 前記重なり領域(10)の長さL (10) と圧縮状態における前記自己拡張型ステント(2)の長さL (2)Comp との比L (10) /L (2)Comp が、圧縮状態において、少なくとも20%である、請求項1に記載の腔内装置。
- 前記径方向突出部(19)の外径が、前記撤回式シース(5)の内径の少なくとも50%、最大で70%である、請求項3に記載の腔内装置。
- 前記径方向突出部(19)の外径が、前記撤回式シース(5)の内径の少なくとも60%である、請求項15に記載の腔内装置。
- 前記角度が、拡張したときの前記保持手段(3)の少なくとも遠位部分において、少なくとも150°である、請求項6に記載の腔内装置。
