Description
この出願は、下記米国仮出願の優先権を主張する:(1)「経カテーテル大動脈弁処置のためのシステムおよび方法」と表題が付けられ2015年4月30日に出願された米国仮出願番号62/155,384号;および(2)「経カテーテル大動脈弁処置のためのシステムおよび方法」と表題が付けられ2015年8月27日に出願された米国仮出願番号62/210,919号。これら仮出願の全体が、参照によって本明細書に取り込まれ、前述の出願日の優先権を主張する。
左総頚動脈、右総頚動脈、左鎖骨下動脈または右鎖骨下動脈のアクセスサイトへの導入に適合する動脈アクセスシース、ここで、該動脈アクセスシースは、該動脈アクセスシースを通して心臓に人工弁を送達するように構成された弁送達システムを受容する大きさおよび形状の内腔を含むものであり;および
該動脈アクセスシース上の、動脈を閉塞するのに適する閉塞要素、
を含んでなるシステムを開示する。
左総頚動脈、右総頚動脈、左鎖骨下動脈または右鎖骨下動脈のアクセスサイトへの導入に適合する動脈アクセスシース、ここで、該動脈アクセスシースは、該動脈アクセスシースを通して心臓に人工弁を送達するように構成された弁送達システムを受容する大きさおよび形状の内腔を有するものであり;および
塞栓保護を提供するための、該動脈アクセスシースに連結されたフィルタ、
を含んでなるシステムを開示する。
左総頚動脈、右総頚動脈、左鎖骨下動脈または右鎖骨下動脈のアクセスサイトへの導入に適合する動脈アクセスシース、ここで、該動脈アクセスシースは、該動脈アクセスシースを通して心臓に人工弁を送達するように構成された弁送達システムを受容する大きさおよび形状の内腔を有するものであり;および
該動脈アクセスシースに流体流通的に接続されたリターンシャント、ここで、該シャントは、該動脈アクセスシースからリターンサイトへの血流路を提供するものである、
を含んでなるシステムを開示する。
左もしくは右総頚動脈または左もしくは右鎖骨下動脈のアクセスサイトへの導入に適合する動脈アクセスシース、ここで、該動脈アクセスシースは、該動脈アクセスシースを通して心臓に人工弁を送達するように構成された弁送達システムを受容する大きさおよび形状の第1の管腔を有するものであり;
該動脈アクセスシースの近位域に配置されたY字型アーム;および
該Y字型アームに流体流通的に接続されたフローシャント、ここで、該フローシャントは、遠位頚動脈を灌流するのに適するものである、
を含んでなるシステムを開示する。
左もしくは右総頚動脈または左もしくは右鎖骨下動脈のアクセスサイトへの導入に適合する動脈アクセスシース、ここで、該動脈アクセスシースは、該動脈アクセスシースを通して心臓に人工弁を送達するように構成された弁送達システムを受容する大きさおよび形状の内腔を有するものであり;
該動脈アクセスシースの側部開口、ここで、該側部開口は、該動脈アクセスシースにその先端から入り該側部開口から出て遠位頚動脈を灌流する血液の順行流を可能にするものであり;および
拡張器、ここで、該拡張器は、該アクセスシースの動脈への挿入中は該アクセスシースの内側にあり、アクセスシース挿入中に血液が該シースを通り前記側部開口から流出するのを阻止するものである、
を含んでなるシステムを開示する。
患者の頚部において、総頚動脈の壁に貫通部を形成し;
該貫通部からアクセスシースを導入し;
該動脈を閉塞し;
該アクセスシースを通して自己大動脈弁に渡してガイドワイヤを挿入し;および
該アクセスシースを通して人工弁を導入し、自己大動脈弁の位置またはその付近において該人工弁を経皮的に展開する、
ことを含んでなる方法を開示する。
患者の頚部において、総頚動脈の壁に貫通部を形成し;
該貫通部からアクセスシースを導入し;
塞栓保護を提供するフィルタを動脈内に展開し;
該アクセスシースを通して自己大動脈弁に渡してガイドワイヤを挿入し;および
該アクセスシースを通して人工弁を導入し、自己大動脈弁の位置またはその付近において該人工弁を経皮的に展開する、
ことを含んでなる方法を開示する。
本明細書の開示内容は、以下の態様を含み得る。
(態様1)
経カテーテル大動脈弁処置のためのシステムであって、
左もしくは右総頚動脈または左もしくは右鎖骨下動脈のアクセスサイトへの導入に適合する動脈アクセスシース、
ここで、該動脈アクセスシースは、該動脈アクセスシースを通して心臓または大動脈に人工弁を送達するように構成された弁送達システムを受容する大きさおよび形状の第1の腔を有し、該第1の腔は、該動脈アクセスシースの近位端において第1の開口を、また、該動脈アクセスシースの最遠位端において遠位開口を有し、該第1の腔の近位端は、止血バルブとY字型アームとを有する近位コネクタを有し、該止血バルブは、その中に前記弁送達システムを通すのに適合する大きさを有するものであり、
ここで、該動脈アクセスシースはさらに、近位端と遠位開口とを有する灌流用の腔を有し、該灌流用の腔は、使用時に、その遠位開口が遠位頚動脈中に配置され遠位頚動脈を灌流することができるような長さを有し、該灌流用の腔の近位端は近位コネクタを有するものである;および
前記灌流用の腔に灌流流体のソースを流体流通的に接続するシャント内腔を規定するシャント、
を含んでなるシステム。
(態様2)
灌流用の腔の遠位開口が放射線不透過マーカーを有する、態様1に記載のシステム。
(態様3)
シャントが、動脈アクセスシースの第1の腔のY字型アームに着脱可能に接続可能な第1の端、および灌流用の腔の近位端に着脱可能に接続可能な第2の端を有する、態様1に記載のシステム。
(態様4)
シャントが、動脈血ソースと灌流用の腔との間に流体通路を形成するように、動脈血ソースに着脱可能に接続可能な第1の端、および灌流用の腔の近位コネクタに着脱可能に接続可能な第2の端を有する、態様1に記載のシステム。
(態様5)
動脈血ソースが、大腿動脈中のシースを含む、態様1に記載のシステム。
(態様6)
灌流用の腔の少なくとも一部が、動脈アクセスシースの第1の腔と平行に接して配置される、態様1に記載のシステム。
(態様7)
シャントが能動的なポンプと組み合わされる、態様1に記載のシステム。
(態様8)
シャントが直列のフィルタをさらに含む、態様1に記載のシステム。
(態様9)
アクセスシースが、その遠位端に閉塞バルーンさらに含む、態様1に記載のシステム。
Claims (4)
- 経カテーテル大動脈弁処置のためのシステムであって、
左もしくは右総頚動脈または左もしくは右鎖骨下動脈のアクセスサイトへの導入に適合する動脈アクセスシース、
ここで、該動脈アクセスシースは、該動脈アクセスシースを通して心臓または大動脈に人工弁を送達するように構成された弁送達システムを受容する大きさおよび形状の第1の腔を有し、該第1の腔は、該動脈アクセスシースの近位端において第1の開口を、また、該動脈アクセスシースの最遠位端において遠位開口を有し、該第1の腔の近位端は、止血バルブとY字型アームとを有する近位コネクタを有し、該止血バルブは、その中に前記弁送達システムを通すのに適合する大きさを有するものであり、
ここで、該動脈アクセスシースはさらに、近位端と遠位開口とを有する灌流用の腔を有し、該灌流用の腔は、使用時に、その遠位開口が遠位頚動脈中に配置され遠位頚動脈を灌流することができるような長さを有し、該灌流用の腔の近位端は近位コネクタを有するものである;および
前記灌流用の腔に灌流流体のソースを流体流通的に接続するシャント内腔を規定するシャント、を含み、
前記シャントは、別の動脈血ソースに接続するように構成された第2のシースに着脱可能に接続された第1の端、および前記灌流用の腔の前記近位コネクタに着脱可能に接続された第2の端を有し、これにより、前記シャントの前記第1の端は前記動脈アクセスシースの前記第1の腔に流体流通的に接続されることはなく、前記シャントは、前記第2のシースを介して前記第1の端が接続された前記別の動脈血ソースと前記第2の端が接続された前記灌流用の腔との間に流体通路を形成する、システム。 - 前記灌流用の腔の前記遠位開口が放射線不透過マーカーを有する、請求項1に記載のシステム。
- 前記灌流用の腔の少なくとも一部が、前記動脈アクセスシースの前記第1の腔と平行に接して配置される、請求項1または2に記載のシステム。
- 前記動脈アクセスシースが、その遠位端に閉塞バルーンさらに含む、請求項1~3のいずれか1項に記載のシステム。
