Description
本願は、2017年11月16日出願の米国仮特許出願第62/587,337号及び2018年7月3日出願の米国仮特許出願第62/693,810号の利益を請求する。これら出願の全体的な内容は参照により本明細書に援用する。
Claims (40)
- 患者を評価及びモニタリングするためのシステムであって、
(i)モニタリングのために選択した身体の場所の初回SEMスキャンを実行する;、
(ii)第一の時間間隔において、初回SEMスキャンにおいて生じ得る損傷を有するとして特定された身体の場所の第二のSEMスキャンを実行する;
(iii)第一の時間間隔よりも長い第二の時間間隔でモニタリングするために選択された全ての身体の場所の第三のSEMスキャンを実行する;
ために構成されたSEMスキャナ、及び
(i)前記初回SEMスキャンから前記身体の場所に関する初回デルタ値を算出する;
(ii)前記初回デルタ値が第一の閾値より大きければ、生じ得る損傷が前記身体の場所にあると判断する;
(iii)患者を、複数のリスク分類を含む群から選択されるあるリスク分類に割り当てることであって、当該割り当てが前記初回デルタ値に一部基づき、割り当てる;
(iv)第二のSEMスキャンのための第二のデルタ値および第三のSEMスキャンのための第三のデルタ値を算出する;
ために構成されたプロセッサ
を含む、システム。 - 前記プロセッサが、さらに、
(i)初回デルタ値が第二の閾値未満であれば、介入を実施しないことを推奨する;及び
(ii)初回デルタ値が第二の閾値以上であれば、介入を実施することを推奨する;
ために構成される、請求項1に記載のシステム。 - 前記介入は、前記身体の場所に一部基づいて、かつ、前記初回デルタ値に一部基づいて、選択される、請求項2に記載のシステム。
- 前記介入は、標準マットレス、高性能マットレス、低摩擦シートカバー、低摩擦パッドのマットレス面、シリコーンパッド、ドレッシング材、ヒールブーツ(heel boot)、保護用クリーム、灌流を高める外用クリーム、第一の間隔で前記患者を体位変換すること、及び第一の間隔よりも短い第二の間隔で前記患者を体位変換することからなる群から選択される、請求項2または3に記載のシステム。
- 前記第一の時間間隔及び前記第二の時間間隔は、割り当てられた前記リスク分類に依存する、請求項1~4のいずれか一項に記載のシステム。
- 前記プロセッサが、さらに、
(i)第二の閾値未満である第二のデルタ値又は第三のデルタ値を有する身体の場所に対して介入を実施しないことを推奨する;及び/又は
(ii)第二の閾値以上である第二のデルタ値又は第三のデルタ値を有する身体の場所に対して介入を実施することを推奨する:
ために構成される、請求項1~5のいずれか一項に記載のシステム。 - 患者を評価及びモニタリングするためのシステムであって、
(i)モニタリングのために選択した1以上の身体の場所の初回SEM測定を実行する;
(ii)第一の時間間隔よりも短い第二の時間間隔で、第一の時間間隔又は第二の時間間隔でモニタリングするために選択された1以上の身体の場所の後続の複数のSEM測定を実行する;
ために構成されたSEMスキャナ、並びに
(i)1以上の身体の場所それぞれについて第一の閾値を設定するために、1以上の身体の場所それぞれについての初回SEM測定のための第一のデルタ値を算出する;
(ii)1以上の身体の場所それぞれについての後続の複数のそれぞれのSEM測定のための後続のデルタ値を算出する;
(iii)後続のデルタ値が第一のデルタ値以上であれば、1以上の身体の場所それぞれについて第一の時間間隔を選択する;
(iv)後続のデルタ値が、第一の閾値より大きい第二の閾値以上であれば、1以上の身体の場所それぞれについて第二の時間間隔を選択する;及び
(v)初回SEM測定後、同一の身体の場所における従前のSEM測定からのデルタ値に基づいた、1以上の身体の場所それぞれについての第一の閾値を設定する;
ために構成されたプロセッサ
を含む、システム。 - 前記リスク分類の群は、低リスクの分類及びリスク有りの分類を含む、請求項1~7のいずれか一項に記載のシステム。
- 前記リスク分類の群は、高リスクの分類をさらに含む、請求項8に記載のシステム。
- 前記割り当てが、リスク評価の結果に一部さらに基づく、請求項1に記載のシステム。
- 患者のケアを管理するためのシステムであって、
(i)入所時に、モニタリングのために選択した1以上の身体の場所の初回SEMスキャンを実行する;
(ii)モニタリングのために選択した1以上の身体の場所の、特定の頻度での、1以上のSEMスキャンを実行する;
ために構成されたSEMスキャナ、並びに
(i)モニタリングのために選択した1以上の身体の場所それぞれについて初回SEMスキャンから初回デルタ値を算出する;
(ii)1以上の身体の場所のいずれかについての初回デルタ値が第一の閾値以上であれば患者が逸脱しているものと判断して介入レベルをN=1に設定する;
(iii)前記第一の閾値以上の第一のデルタ値を有する1以上の身体の場所それぞれに対して、レベル-Nの介入を実施することを推奨する;
(iv)レベル-Nの頻度で、1以上のSEMスキャンの特定の頻度を設定する;
(v)モニタリングのために選択した1以上の身体の場所それぞれについて1以上のSEMスキャンについての新規デルタ値を算出し、前記新規デルタ値に基づいて、
(1)同一の身体の場所において、従前のデルタ値以上の新規デルタ値を有する、1以上の身体の場所それぞれについてレベルN+1の介入を実施することを推奨し、レベルN+1の頻度で、1以上のSEMスキャンの特定の頻度を設定する;
(2)同一の身体の場所において、第一の閾値以上であるが従前のデルタ値未満の新規デルタ値を有する、1以上の身体の場所それぞれについてレベルNの介入を実施することを推奨し、レベルNの頻度で、1以上のSEMスキャンの特定の頻度を設定する;又は
(3)第一の閾値未満の新規デルタ値を有する、1以上の身体の場所それぞれについてレベルN-1の介入を実施することを推奨し、レベルN-1の頻度で、1以上のSEMスキャンの特定の頻度を設定する;
ために構成されたプロセッサ
を含む、システム。 - 患者が転所又は退所となる場合に、システムが、さらに、
(i)入所時に、モニタリングのために選択した1以上の身体の場所の最後の組のSEMスキャンを実行する;
(ii)モニタリングのために選択した1以上の身体の場所それぞれに対して、最後の組のデルタ値を算出する;及び
(iii)前記最後の組のデルタ値を文書化する;

- 褥瘡性潰瘍の処置を必要とする患者を特定及び処置するためのシステムであって、
(a)SEM測定を行うために構成された、表皮下水分(SEM)スキャナ、

(c)プロセッサに電子的に連結され、それに格納された、プロセッサ上で実行された場合、以下のステップ:

(1)第一のサブセットのSEM測定の一部から第一の場所の第一のデルタ値、及び
(2)第二の場所における少なくとも1つのさらなるサブセットのSEM測定の一部から第二の場所の第一のデルタ値
を算出するステップ;
(iii)第一の場所の第一のデルタ値が第一の場所の第一の閾値を超えるか否かを判断するステップ、及び第二の場所の第一のデルタ値が第二の場所の第一の閾値を超えるか否かを判断するステップ;
(iv)第一の場所の第一のデルタ値が第一の場所の第一の閾値を超えていなければ第一の場所に特有のレベル-0の介入を施与するステップ、及び第二の場所の第一のデルタ値が第二の場所の第一の閾値を超えていなければ第二の場所に特有のレベル-0の介入を施与するステップ;及び
(v)第一の場所の第一のデルタ値が第一の場所の第一の閾値を超えていれば第一の場所に特有のレベル-Nの介入を施与するステップ、及び第二の場所に特有の第一のデルタ値が第二の場所の第一の閾値を超えていれば第二の場所に特有のレベル-Nの介入を施与するステップであって、ここでNは整数であり、かつ1以上の値を有する、ステップ;
を実行する命令を含む、非一時的な記録媒体
を含む、システム。 - 患者が褥瘡性潰瘍の視認可能な徴候を有していない、請求項16に記載のシステム。
- Nが1に等しい値を有する、請求項16に記載のシステム。
- Nの値が、第一のデルタ値が第一の閾値を超えた量に基づいて、2以上である、請求項16に記載のシステム。
- Nが10を超えない値である、請求項16に記載のシステム。
- レベル-Nの第一の場所に特有の介入がレベル-0の第一の場所に特有の介入よりも効果的な介入である、請求項16に記載のシステム。
- 第一の場所の第一のデルタ値が、つま先、踵、仙骨、脊椎、肘、肩甲骨、後頭部、坐骨結節、胸骨、耳、及び患者の他の肉づきのよい組織からなる群から選択される、患者の身体の場所において得られた第一の複数のSEM測定の一部、から算出された、請求項16に記載のシステム。
- 褥瘡性潰瘍の処置を必要とする患者を特定及び処置するためのシステムであって、
(a)SEM測定を行うために構成された、表皮下水分(SEM)スキャナ、
(b)SEMスキャナに電子的に連結され、かつSEM測定を受領するために構成されたプロセッサ、並びに
(c)プロセッサに電子的に連結され、それに格納された、プロセッサ上で実行された場合、以下のステップ:
(i)患者の第一の複数の表皮下水分(SEM)測定を受領するステップ;
(ii)施与される介入レベルに対応する第一の予め決定された頻度で、患者に第二の複数のSEM測定を受領するステップ;
(iii)第一の複数のSEM測定の一部から第一のデルタ値を算出するステップ;
(iv)第二の複数のSEM測定の一部から第二のデルタ値を算出するステップ;
(v)第一のデルタ値が第一の閾値を超えるか否かを判断するステップ;
(vi)第二のデルタ値が第二の閾値を超えるか否かを判断するステップ;
(vii)第一のデルタ値が第一の閾値を超えていればレベル-Nの第一の介入を施与するステップ;
(viii)第二のデルタ値が第二の閾値を超えていなければ第一の介入の施与を継続し、ここでNは整数であり、かつ1以上の値を有する、ステップ;
(ix)第二のデルタ値が第二の閾値を超えていなければ第一の予め決定された頻度で複数のSEM測定を受領することを継続するステップ;
(x)第二のデルタ値が第二の閾値を超えていればレベル-Mの第二の介入を施与するステップであって、Mはある整数であり、かつMはNより大きい、ステップ;及び
(xi)第二のデルタ値が第二の閾値を超えていればレベル-Mに対応する第二の予め決定された頻度で複数のSEM測定を受領するステップ;
を実行する命令を含む、非一時的な記録媒体
を含む、システム。 - 第二の閾値が第一の閾値と同一である、請求項23に記載のシステム。
- 第二の閾値が第一の閾値より大きい、請求項23に記載のシステム。
- MがN+1と等しいが、10を超えない値を有する、請求項23に記載のシステム。
- Mの値が、第二のデルタ値が第二の閾値を超えた量に比例する、請求項23に記載のシステム。
- 前記非一時的な記録媒体が、以下のステップ:
(i)第二のデルタ値が第三の閾値未満であるか否かを判断するステップ;
(j)第二のデルタ値が第三の閾値未満であれば、かつ、第一の介入がレベル-0のものでなければ、レベル-(N-1)の介入を施与するステップ;及び
(k)第二のデルタ値が第三の閾値未満であれば、レベル-(N-1)に対応する予め決定された頻度で複数のSEM測定を受領するステップ;
をさらに実行する、請求項23に記載のシステム。 - レベル-0の介入が、良好な栄養、標準マットレス、の提供、24時間毎の体位変換、及びこれらの組合せからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-1の介入が患者にヒールブーツを提供すること、患者をウェッジ(くさび)により体位変換すること、及び/又は患者の仙骨を乾いた状態に維持することからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-2の介入が患者の支持面を変更すること、及び患者のマットレスを体圧分散マットレスに変更することからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-3の介入が患者の踵の後方又は側面にドレッシング材を付与すること、及び患者の仙骨の上にドレッシング材を付与することからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-4の介入が患者のシートカバーを低摩擦シートカバーに変更すること、及び患者のマットレスをダイナミックマットレスに変更することからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-5の介入が患者の下腿に対して低摩擦パッドのマットレス面を提供すること、及び保護用クリームを患者の仙骨に塗布することからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-6の介入が現在提供されているよりも短い間隔で患者を体位変換すること、及び患者の仙骨に神経筋刺激を付与することからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-7の介入が保護用クリームを患者の踵に塗布すること、及び外用クリームを患者の仙骨に塗布して灌流を高めることからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-8の介入が患者の踵に対して神経筋刺激を付与すること、及び患者の身体の下にシリコーンパッドを提供することからなる群から選択される、請求項16又は23に記載のシステム。
- レベル-9の介入が外用クリームを患者の踵に塗布して灌流を高めることである、請求項16又は23に記載のシステム。
- レベル-10の介入が患者の下腿に対してシリコーンパッドを提供することである、請求項16又は23に記載のシステム。
- 前記SEMスキャナが、静電容量を測定するために構成された1以上のセンサーを含み、かつ前記静電容量がSEM測定に変換される、請求項16又は23に記載のシステム。
