Description
血管アクセス機器(VAD)は、一般的に使用される治療機器であり、静脈内(IV)カテーテルが含まれる。VADには、末梢カテーテルと中心静脈カテーテルという2つの一般的な分類がある。細菌やその他の微生物は、液体や医薬品を供給するための補助人工心臓への接続時に、アクセスハブ、ポート、バルブから患者の血管系に侵入する可能性がある。各アクセスハブ、ポート、バルブ、または接続部は、カテーテル関連血流感染(CRBSI)を引き起こす危険性があり、これはコストがかかるだけでなく、致命的となる可能性がある。CRBSIを減少させ、VADの正しい使用とメンテナンスを確実にするため、消毒と洗浄の手順を含む実施基準が開発された。消毒用キャップは、米国医療疫学会(SHEA)ガイドラインおよび輸液看護師基準(INS)ガイドラインに追加されている。
本開示の第1態様は、ハウジング、ボール、消毒剤又は抗菌剤、及び取り外し可能なシールを含むキャップに関連する。ハウジングは、頂部壁、キャビティを形成する外表面及び内表面を有する本質的にシリンダ状の側壁、及び雄ルアーコネクタのハブを受容するためのハウジング内のキャビティへの開口部を有するシリンダ状の側壁によって形成された開口底部を含むことができる。
本開示の実施形態は、ネジ付きコネクタを含む医療用コネクタに接続して消毒するための消毒用キャップに関するものである。コネクタは、雄ルアーコネクタであってもよいし、雌ルアーコネクタであってもよい。本開示は、医療用コネクタの流体経路またはルーメンへの消毒剤の侵入を防止しながら、コネクタまたは継手の周辺または周囲の効果的な消毒を提供するための機構を提供することを目的とする。ここに開示される消毒用キャップは、雄ネジまたは雌ネジのコネクタと共に利用され得ることが企図される。

- ハウジングが、

前記キャビティ内に配置されたボールであって、前記ボールが、雄ルアーコネクタの接続前の初期状態において、前記キャビティの前記内表面とのシールを提供して、殺菌剤または抗菌剤が前記キャビティから漏れるのを防ぐように構成されている、ボール、
前記殺菌剤または前記抗菌剤、
前記シリンダ状の側壁によって形成されて前記雄ルアーコネクタのハブを受容するために前記ハウジング内で前記キャビティの開口部を有する開口底部、および
前記シリンダ状の側壁の外表面上の外ネジ部であって、前記外ネジ部が前記雄ルアーコネクタのかみ合い特徴部とかみ合うのに十分である、外ネジ部、

前記キャビティが、前記ボールの直径よりも小さい第1直径D1を有する真っ直ぐなシリンダ状の側壁を有する前記開口底部に隣接する第1部分と、前記第1部分に隣接して配置された第2テーパ部分であって、前記開口底部から前記キャビティの前記頂部壁に向かう方向に増加する第2直径D2を有する第2テーパ部分と、前記キャビティの中心軸に向けて突出する複数のリブを有する真っ直ぐなシリンダ状の側壁を有する第3部分であって、前記キャビティの前記内表面の前記第3部分が前記第2テーパ部分に隣接して配置され前記ボールの直径と同じかそれより大きい直径D3を有する、第3部分と、を有する、キャップ。 - 前記雄ルアーコネクタのハブが前記キャビティの前記内表面内に受容されるとき、前記雄ルアーコネクタの前記ハブ上のかみ合い特徴部に前記外ネジ部の少なくとも一部をかみ合わせることにより、前記ハブが前記キャビティの前記内表面内に固定される、請求項1に記載のキャップ。
- 前記キャビティの前記開口底部に取り付けられた取り外し可能なシール又はセプタムを更に備え、前記キャップの使用前に前記殺菌剤又は前記抗菌剤を前記キャビティ内に維持するためのシールを形成する、請求項1に記載のキャップ。
- 前記側壁の外壁面が、複数の把持部材を含む、請求項1に記載のキャップ。
- 前記雄ルアーコネクタとの接続時に、前記ボールの前記ハウジングの前記頂部壁へ向けた圧縮が生じる、請求項1に記載のキャップ。
- 前記ボールの圧縮が前記雄ルアーコネクタを消毒する、請求項5に記載のキャップ。
- 前記ボールは、前記キャビティの前記内表面上のリング状のくぼみによって半径方向の圧縮下にあり、前記ボールを前記キャビティ内に保持している、請求項1に記載のキャップ。
- 前記殺菌剤または前記抗菌剤が、イソプロピルアルコール、エタノール、2-プロパノール、ブタノール、メチルパラベン、エチルパラベン、プロピルパラベン、没食子酸プロピル、ブチルヒドロキシアニソール(BHA)、ブチルヒドロキシトルエン、t-ブチル-ヒドロキノン、クロロキシレノール、クロロヘキシジン、二酢酸クロロヘキシジン、グルコン酸クロロヘキシジン、ポビドンヨード、アルコール、ジクロロベンジルアルコール、デヒドロ酢酸、ヘキセチン、トリクロサン、過酸化水素、コロイド銀、塩化ベンゼトニウム、塩化ベンザルコニウム、オクテニジン、抗生物質、およびこれらの混合物から本質的になる群から選択される、請求項1に記載のキャップ。
- 前記殺菌剤または前記抗菌剤が、グルコン酸クロロヘキシジンおよび二酢酸クロロヘキシジンのうちの少なくとも1つを含む、請求項8に記載のキャップ。
