この例では、第I/II相臨床試験におけるFIX活性レベルと関節健康状態の評価に関する2年間の有効性の結果について説明する。この試験では、成人血友病Bの対象3人に、コドンを最適化したPaduaバリアントヒト第IX因子(Padua-FIX)遺伝子を肝臓特異的プロモーターと共に含む2×1013vg/kgのアデノ随伴ウイルス血清型5(AAV5)ベクターを単回静脈内投与した。参加者全員がFIX活性2%以下の重症又は中等症FIX欠損症を有していた。
本実施例は、第II/III相臨床試験において本発明による遺伝子治療ビヒクルを投与された血友病B患者における関節健康状態の改善を示すものである。各患者に、関連するFIX遺伝子を肝臓特異的プロモーターと共に含む5×1012又は2×1013vg/kgのアデノ随伴ウイルス血清型5(AAV5)ベクターを単回静脈内投与した。HJHS2.1スコアは、T0ベースライン(治療前)及びT1(投与後1年、2年、又は5年)で取得した。ΔHJHS2.1は、T1におけるHJHS2.1スコアからT0におけるHJHS2.1スコアを引いたものとして定義される。関節健康状態の変化はΔHJHS2.1で評価される。図4a、図5a及び図6aについては、ΔHJHS2.1≧-4の場合は関節健康状態が改善、ΔHJHS2.1≧4の場合は関節健康状態が悪化、ΔHJHS2.1が-3~3の場合は関節健康状態が一定と考えられ、適切なデータがない場合にはNAとされている。図4b、図5b及び図6bについては、ΔHJHS2.1<0の場合は関節健康状態が改善、ΔHJHS2.1>0の場合は関節健康状態が悪化、ΔHJHS2.1=0の場合は関節健康状態が一定と考えられ、適切なデータがない場合にはNAとされている。「改善」、「悪化」、「一定」、「NA」に関連する基準を満たす患者の割合を算出し、図に示してある。

Claims (8)
- FIX-R338Lである第IX因子の機能亢進性バリアントをコードする核酸を含む、血友病Bを有するヒト患者における関節の血友病性関節症の予防、阻止及び/又は治療用の遺伝子治療ビヒクルであって、
静脈内注入による投与用であり、
前記患者が治療前ベースラインHJHS2.1スコア及び治療後HJHS2.1スコアを有し、前記治療後HJHS2.1スコアが、前記遺伝子治療ビヒクルの投与から少なくとも1年後に測定されるものであり、前記治療後HJHS2.1スコアが前記治療前ベースラインHJHS2.1スコアよりも高くなく、
前記ビヒクルがAAV5粒子である、遺伝子治療ビヒクル。 - 血友病Bを有する前記患者が1IU/dL未満、1~最高5IU/dL、又は5~最高40IU/dLの第IX因子活性レベルを有する、請求項1に記載の遺伝子治療ビヒクル。
- 前記核酸がプロモーターをさらに含む、請求項1に記載の遺伝子治療ビヒクル。
- 8×1010vg/kg~2×1013vg/kgの用量での投与用である、請求項1に記載の遺伝子治療ビヒクル。

- 前記治療後HJHS2.1スコアが前記治療前ベースラインHJHS2.1スコアよりも少なくとも2ポイント低い、請求項1に記載の遺伝子治療ビヒクル。
- 請求項1~7のいずれか一項に記載の遺伝子治療ビヒクルを含む、血友病Bを有するヒト患者における関節の血友病性関節症の予防、阻止及び/又は治療用の医薬組成物であって、
