Description

【発明の詳細な説明】 本発明は移植により関節軟骨の欠陥と骨の欠陥
とを修復するに使用するための組成物に関するも
のである。本発明の組成物はそうした修復用に使
用できる。それらは、それとして、または天然ま
たは人口骨をキヤリヤとして一諸にして、ゲルの
形で提供される。以下の記述は主として関節軟骨
の修復に関連しているが、これは最も問題であり
繊細な生物学構造であるからである。本発明の新
規な組成物は、軟骨形成誘因因子の影響により潜
在的に軟骨細胞に変換されうるところの萌芽性の
軟骨細胞ないし或る間葉織細胞を組合せて含んで
居り、細胞は適量の線維素原、トロンビン、及び
アンチプロテアーゼを組合せて膠として役立つよ
うに作られている。結果としてできるゲルは組織
培養貯蔵条件下に長期間に渉つて貯蔵でき、輸送
でき、容易に取扱いできる。
本発明の好ましい態様によれば、細胞外基質
（ECM）、または、SM，FGF，CGF，BDGFの
如き或る生長フアクターまたはこれらの何れかの
組合せをそうしたゲル中に内蔵させられうる。骨
の或る欠陥の修復のためには、そうしたゲルで満
して適当な形状にした骨部材を含む移植物を使用
しうる。
本発明の他の更に別の特徴は今後明かとなるで
あろう。
関節軟骨が外傷、感染ないし退化性操作によつ
て混ぜられると、そうした損傷は一般に治し得
ず、改良さえもできない。これまで、骨軟骨植接
にたよることや、種々の形の補綴を用意すること
に種々の試みが行われて来たが、長期的結果は貧
弱であり落胆すべきものであつた。軟骨移植の源
泉として培養した軟骨細胞を使用する試みも報告
されたが移植物と隣接する軟骨との一体化は一般
に不満足であつた。
本発明によれば、直ちに使用可能な組成物が提
供され、萌芽性の軟骨細胞または、間葉織起源の
細胞、何等かの種類のもので、軟骨形成誘因フア
クターの影響により、試験管内または生体内で軟
骨細胞へ変換されうるもので、“ゲル” を形成する適当な生物学的媒体中で大人の関節軟
骨及び骨への損傷の修理に対して、損傷の場所へ
の移植により行うのに使用しうるものが提案され
ている。また、そうした移植可能材料の生産用の
操作もまた提案されている。
また、軟骨の欠陥の修理に使用する組成物も提
案されている。その組成物は、適当な萌芽性の組
織の機械的破砕と共にトリプシン化による萌芽性
軟骨細胞（若さを留めた軟骨細胞）の遊離で作
り、ついで適当な媒質上で培養し、細胞を収穫
し、それらを線維素原、アンチプロテアーゼ及び
トロンビンを加えてその結果として、培養器中で
限られた時間の期間貯蔵ができるゲルを得、また
は、収獲された細胞が望むゲルへ使用する前に深
冷及び氷解によつて長期間保存されうるようにな
るようなゲルを得る。ゲルは、欠陥を満すように
トロンビン溶液を噴霧した移植の場所に適用する
前に線維素原の溶液中に浸されている。動物実験
（鳥類及び哺乳類種につき）に於いて、軟骨と骨
との欠陥は２乃至12ケ月の期間後の検査したとと
ころでは、初期損傷の優れた修復を示して居て、
正しく満されたことが示された。
本発明の組成物は、外傷ないし老齢のせいでの
人間関接軟骨または骨の損傷の修復に価値があ
る。そうした損傷は種々の型の軟骨及び骨で、接
合部の退化性傷害や破砕を含んでいる。
本発明の好ましい態様によれば、本発明の新規
な処方は、以下に於て開示される如くに特殊技法
で調製される萌芽性軟骨細胞の或量の細胞外基質
（ECM）を含んでいる。
ECMを設けることで、新規な組成物が植付け
られた時に軟骨細胞の生長を高めるが、これらが
望む天然環境に似ている事が最も確からしい。
軟骨細胞は、移植される材料が適用さるべき種
の適当な萌芽性骨端組織から得られる。異質形成
細胞は、HLAタイプ化にたよる必要なしに全く
満足な結果を与える。人間での使用に対しては、
人間萌芽性軟骨細胞が培養された。
毎単位容積当りの軟骨細胞数は或る値を超えな
いものであるべきである。それでないと、細胞の
類壊死が生じ、より劣る結果が得られる。軟骨細
胞濃度の代表的値は、ゲルの毎ミリリツトル当り
約100000から500000である。毎ミリリツトル当り
約５から50単位のトロンビンと約25から80mg／ml
線維素原が使用される。一般にプロテアーゼ抑制
剤を、ゲルの速い溶解を防ぐか、抑えるため組成
物へ加える。天然または合成プロテアーゼ抑制剤
を使つてもよい。適切なプロテアーゼ抑制剤は、
ゲルの約10から20mg／mlの量で使用されるε−ア
ミノカプロイン酸、ゲルの約１から２mg／mlの量
で使用されるトランエキゼミン酸の如きである。
アンチ・トリプシン（チキン卵白、シグマ、第
型）の如き天然プロテアーゼ抑制剤を使つてもよ
い。ゲルの遅い形成が望まれる時は、５から10単
位毎mlの程度の量のトロンビンが使用され、ゲル
の速い硬化が望まれる時は20から50単位毎mlの程
度の量のトロンビンが使用される。
上に加えて、ゲル中に、局部型（例えばECM，
BMP）またはホルモン性、例えば、ソマトメジ
ン（SM）状ペプチード；軟骨成長フアクター
（CGF）；などの一つ以上の生長フアクターを内
蔵させると有利である。生長フアクターは、下記

mg／ml・；BDGF；100マイクログラム／ml程度
に；CGF：２，３ナノグラム／ml；SM：２，３
ナノグラム／ml。
Ｆ−12プラス10％子牛胎児血清で作つた組成物
は、約37℃でCO2培養器内に二、三週の間貯蔵で
きる。本発明によるゲルは、直ちに使用でき、下
記に示す如くに、損傷された、または、欠陥個所
に直接に適用しうる。
或る修復に対しては、トロンビン、アンチプロ
テアーゼ、線維素原及び一つ以上の生長フアクタ
ーおよび／または上に規定された型の細胞の小濃
度を伴うか、または、そうした細胞なしでさえ
も、ホルモンから成るゲルで満した骨構造（天然
または人工）から成る移植を利用することが推奨
される。後者の場合、環境からの細胞が移植内に
徐々に滲透し、環境と移植との間に凝集性一体物
を形成する。
本発明を下記の例を参照して描写するが、これ
は非制限的な具合に解釈さるべきものである。
例１：軟骨修復用組成物の調製 出発材料として長い骨（脛骨、大腿骨）の骨端
を使用した。
萌芽性軟骨細胞（若さを留めた軟骨細胞）の遊
離操作は、骨端の激しいトリプシン化から成つて
いる（１％のトリプシンを45分間、37℃の水浴中
で回転振盪機にかけて培養し、同時に、手動チヤ
ンネル付テフロンホモジエナイザーにより一定の
機械的破砕をする。）トリプシン活性は、反蛋白
質分解性物質を含む血清により停止する。出来上
つた単一細胞懸濁液は、軟質寒天（Ham−Ｆ−
12含有0.6％寒天）を塗布した板上の液体Ham
Ｆ−12内で数日（６−10日）種除きさせる。この
生長期間の間、線維芽細胞の大抵のものは死に去
り、軟骨細胞増殖が起る。軟質寒天板からの細胞
は更にスピンナー壜内のＦ−12の懸濁培養内に移
され、更に３−10日の期間生長させられる。スピ
ンナー壜内の生長条件は再び線維芽細胞よりも軟
骨細胞を好んで支持する。スピンナー壜からの細
胞は遠心分離により採取され、直接に使用される
か、または、液体窒素中にて長期間冷凍保存され
る（20％胎児子牛血清、10％ジメチルスルフオキ
サイド（DMSO）及び70％Ｆ−12）。出来上つた
軟骨細胞のペレツトは、線維素原（50mg／ml）ま
たは如何なる他の血清凝結蛋白及びトリプシン抑
制剤（50μ／mlシグマ型）または他のアンチプ
ロテアーゼを含む小容積の燐酸塩緩衝塩水中に再
懸濁される。
細胞（培養の期間と大きさとに従つて濃度が１
−５×105細胞／mlの間に渉つている）と、線維
素原とトリプシン抑制剤とを含んでいる最後の溶
液を溶液Ａとする。（ミクロテスト板（蓋付き96
Ｆ Nuncデンマーク）または、多岐井戸に、ト
ロンビン溶液（溶液Ｂ）（40mM CaCl2の1U／30
ml）の30マイクロリツトルを各井戸へ加え（底及
び側壁上に平均に分布して）、それから溶液Ａを
90マイクロリツトル加え、そして混合物を固化
（ゲル化）させる。
需要に従つて、最後の量は変化出来、かくて、
溶液ＡとＢの比を３：１（ｖ／ｖ）に保つ。Ｆ−
12の0.2mlで蔽つたゲルはそれから或期間（約14
日）、５−10％CO2を空気に入れて平衡化した培
養器中か、または冷凍保存されて、保たれること
ができる。
移植では、損傷した場所をトロンビン溶液で繊
細に噴霧し、他方、移植に先立つゲルは線維素原
とトリプシン抑制剤を含む（それぞれ50mg／ml及
び50μｇ／ml）溶液中に浸漬し、そして試料を損
傷した場所に圧入し欠陥を滑らかに満す。
萌芽性軟骨細胞の代りに、萌芽性間葉織細胞
（段階24）または骨髄幹細胞を使つてもよい。加
うるに、トラツプされた非差異化間葉織細胞をつ
けた如何なる成人連結組織を使用してもよく、培
養中に細胞を発生し、それが最後に自己差異化に
より軟骨細胞に形成され、または軟骨細胞フアク
ターにより指向される。
先天性移植に対しては、受容体の成人組織内に
トラツプされた同質性間葉織型細胞で、皮膚線維
芽細胞、生検により得られる骨髄細胞の如きを使
つてもよく、これらは、培養中に細胞を生じ、そ
れらが軟骨細胞に変換される。
例２：ゲル含有ECM（細胞外基質） ａ EMCの調製 スピンナー壜（14−21日培養中）からの萌芽性
ひよこ軟骨細胞を、２×105細胞／35mm皿の初期
濃度で板にし、フイブロネクチンを塗布した皿上
にアスコルビン酸（50μｇ／ml）で補足したＦ−
12媒質中に維持した。培養らは６日以内に合流に
なり、それから媒質を更新し、また培養を更に追
加の６日間培養した。三重の培養の組はトリプシ
ン化され、Conlter計数器（モデル工業Ｄ）によ
り細胞計数を調べる為に使用した。末端での細胞
計数は１×106細胞／板の周りの範囲であつた。
総ての他の板をそれから燐酸緩衝塩水（PBS）
で洗浄し、20分間蒸溜水中で20mM NH4OHに
曝した。そして３回PBSで再洗滌し、蒸溜水で
２回最終洗滌し、細胞体質ないし核が、皿に塗布
したもとのままのECMと組合されて認められる
ことがないようにした。ECMはゴムポリスマン
で採集して凍結真空乾燥した。乾燥粉末化ECM
の収率は0.3−1.0mg／板の間の範囲であつた。
ECMは５乃至20mg毎リツトルの量で混ぜ合せ
た。最良の結果は約10mg／mlで得られた。
軟骨細胞ゲルを種々の種（鳥類及び哺乳類）内
の移植として実験し、巨視的観察、組織学的断面
及び生化学試験により採集したデータは、移植の
場所に於て、二三ケ月以内に、欠陥は活性の増殖
する軟骨細胞で正しく満され（関節軟骨の表面を
含めて、環境に調整されて）、特徴的な（表現型
の）代謝性体質と古い隣接軟骨組織と積重つた井
戸（線維軟骨、または他の軟かい組織がヘリに無
い）とを表していた。

Claims (1)

  1. 【特許請求の範囲】 １ 移植によつて関節の軟骨および骨の欠陥を修
    復するためのものであつて、それの組成物は、そ
    の組成物が移植されるべき種の萌芽性軟骨細胞ま
    たは間葉織細胞で、線維素原、トロンビン、及び
    天然ないし化学的プロテアーゼ抑制剤の或る量と
    から成る生物学的膠による軟骨形成誘因因子によ
    り軟骨細胞に変換されうるものを含んでおり、毎
    ミリリツトルごとに、100000から500000個の軟骨
    細胞または間葉織細胞を含んでいる組成物。 ２ ５乃至20mg／mlの細胞外基質（ECM）を含
    んでいる特許請求の範囲第１項記載の組成物。 ３ 局部的生長ないしホルモン生長因子剤を含ん
    でいる特許請求の範囲第１項記載の組成物。 ４ 局部的生長因子が軟骨起源生長因子（CDF）
    または骨起源生長因子（BDGF）である特許請求
    の範囲第３項記載の組成物。 ５ ホルモン性生長因子が血清または下垂体から
    のソマトメジンである特許請求の範囲第３項記載
    の組成物。 ６ 細胞が軟骨細胞または未分化間葉織捕捉細胞
    を含む結合組織から導出された細胞、培養中に軟
    骨細胞になるように増殖し変換されるよう誘導さ
    れる細胞である特許請求の範囲第１項記載の組成
    物。 ７ ゲルが、適当な形状にした親水化された（キ
    ール骨）または人工的骨中に包埋されている特許
    請求の範囲第６項記載の組成物。 ８ 移植によつて関節の軟骨および骨の欠陥を修
    復するためのものであつて、それの組成物は、そ
    の組成物が移植されるべき種の萌芽性軟骨細胞ま
    たは間葉織細胞で、線維素原、トロンビン、及び
    天然ないし化学的プロテアーゼ抑制剤の或る量と
    から成る生物学的膠による軟骨形成誘因因子によ
    り軟骨細胞に変換されうるものを含んでおり、毎
    ミリリツトルごとに、100000から500000個の軟骨
    細胞または間葉織細胞を含んでいる組成物を欠陥
    組織内に植付けることを含む骨端軟骨及び骨修復
    の方法。 ９ 細胞外基質（ECM）または適当な局部的な
    いしホルモン性生長型因子で、組成物が移植され
    るべき種の萌芽性軟骨細胞または間葉織細胞で、
    線維素原、トロンビン、及び天然ないし化学的プ
    ロテアーゼ抑制剤の或る量とから成る生物学的膠
    による軟骨形成誘因因子により軟骨細胞に変換さ
    れうるものを含むか、またはそうした細胞を欠い
    ているものと共に、トロンビン、アンチプロテア
    ーゼ及び線維素原とから成る生物学的ゲルで含浸
    された適当に形状づけられた骨構造を含む、軟骨
    ないし骨修復用の移植体。
